雷替曲塞腹腔灌注化疗在进展期贲门癌治疗中的应用研究

摘　　要：目的探讨雷替曲塞腹腔灌注化疗在手术治疗局部进展期贲门癌中的应用价值。方法根据治疗方法不同将进展期贲门癌患者60例分为2组,治疗组30例行手术治疗并在术中腹腔灌注雷替曲塞,对照组30例给予常规手术治疗。观察2组术前及术后白细胞计数、肝肾功能、细胞免疫指标变化,统计2组胸腔引流时间及胃肠道功能恢复时间,比较2组围术期并发症及不良反应发生情况,随访2组1,3,5年生存率。结果 2组术前及术后白细胞计数、肝肾功能及各项细胞免疫指标比较差异均无统计学意义(P均>0.05);术后2组胸腔引流时间、胃肠道功能恢复时间、并发症及不良反应发生率比较差异均无统计学意义(P均>0.05),治疗组1,3,5年生存率均明显高于对照组(P均<0.05)。结论雷替曲塞术中腹腔灌注化疗能够提高局部进展期贲门癌患者的长期生存率,且安全性高,值得推广应用。Objective It is to investigate the application value of raltitrexed intraperitoneal perfusion chemotherapy during operation of advanced gastric cardial carcinoma. Methods 60 patients with local advanced gastric cardial carcinoma were di- vided into two groups according to the treatment method, 30 cases in treatment group were treated with rahitrexed intraperitone- al perfusion chemotherapy during the radical operation, whereas the remaining 30 cases in control group underwent surgical treatment only. The liver and kidney functions, WBC, immunity index which measured on the 1 day of pre-operation and the 7th day of post-operation respectively were compared. The time of abdominal drainage and gastrointestinal function recovery in the two groups were observed. The other aspects such as the operative complication, the toxicity of chemotherapy, and the mortality and survival rate between these two groups were compared. Results There were no significant differences in liver and kidney functions, WBC and immunity index between the two groups in pre-operation and post-operation （ all P 〉 0.05 ） ; and there were no significant differences in the operative complication incidence, the toxicity of chemotherapy between the two groups （ all P 〉 0.05 ）. The survival rate of 1 - , 3 - , 5 - year in the treatment group were markedly higher than those in the control group （ all P 〈 O. 05 ）. Conclusion Raltitrexed intrapertioneal perfusion chemotherapy during operation can improve the long-term survival rate of patients with local gastric cardial cancer, it is safe, and is worthy of popularization and application.
