基层医院超声乳化术治疗急性闭角型青光眼的体会

摘　　要：目的观察基层医院行白内障品状体超声乳化吸出术联合人工晶体植入术治疗原发性急性闭角型青光眼的效果。方法回顾性研究。晶状体超声乳化吸出术治疗原发性急性闭角型青光眼15例（18眼）。男6例（6眼），女9例（12眼）。年龄55～75岁，平均（65．53±6．38）岁。术后随访3～6个月，观察手术后视力、眼压的变化及并发症。结果术后视力均有不同程度的提高（P〈0．05）。眼压：术前眼压平均为（24．1±4．4）mmHg，术后眼压平均为（12．6±2．7）mmHg（1mmHg=0．133kPa）术前术后比较，差异有统计学意义（P〈0．01）。主要并发症有：角膜水肿、虹膜炎症反应和视觉紊乱。结论晶状体超声乳化术治疗急性闭角型青光眼可解除瞳孑L阻滞、控制眼压、提高视力，临床疗效佳，并发症少且容易处理，适合基层医院开展。Objective To observe the clinical efficacy of phacoemulsification combined with intraocular lens implantation for the treatment of acute angle closure glaucoma in primary hospital. Methods A retrospective study. Eighteen eyes of 15 cases of acute angle closure glaucoma were treated by phacoemulsification combined with intraocular lens implantation. Six patients （ 6 eyes ） were male and 9 patients were female. The mean age was （65.53 ＋_ 6.38） years with a range from 55 to 75 years. After the surgery. , the patients were followed up for 3-6 months and the changes of visual acuity,intraocular pressure （lOP） and postoperative complications were recorded. Results The postoperative visual acuity of all patients improved（P 〈 0. 05 ）. The average preoperative IOP was （24.1±4.4） mmHg, while the average postoperative lOP was （ 12.6 ±2. 7 ） mmHg. The difference in IOP before and after the operation was statistically significant（ P 〈 0.01 ）. Main complications included corneal edema, iris inflammation and visual disorders. Conclusion Phacoemulsification for acute angle closure glaucoma can relieve pupillary block, control the intraoeular pressure and improve visual acuity. This treatment can be used in primary or county hospital because of less complications and good clinical effect.
