摘 要:目的探讨软组织肉瘤(STS)患者治疗前18F-FDGPET-CT最大标准摄取值(SUVmax)的预后价值。方法回顾性分析34例经病理确诊的STS患者资料,所有患者于治疗前行18F-FDGPET-CT检查,采用Mann—Whitney非参数检验及Spearman相关性检验评价SUVmax与预后因子的关系。采用Kaplan—Meier法进行生存分析,COX模型进行多因素分析。结果34例STS中位SUVmax为10.3(1.5。28.2),肿瘤最大直径为6.7cm(1.8—17.2cm),两者呈正相关(r=0.389,P=0.028)。SUVmax与病理学分级、美国癌症联合委员会(AJCC)分期及远处转移均有相关性(均P〈0.05)。单因素分析结果显示,远处转移、病理学分级、AJCC分期及SUVmax与STS患者的生存相关(均P〈0.05),而年龄、性别、发病部位及肿块大小与生存均无相关性(均P〉0.05)。多因素分析显示,远处转移及SUVmax是影响患者生存的独立危险因素(均P〈0.05)。结论STS患者治疗前。18F-FDGPET-CT的SUVmax与患者的预后因子间具有较好的相关性,能够预测患者的预后。Objective To determine the prognostic value of maximum standard uptake (SUVmax) of pretreatment 18F-FDG PET-CT in patients with newly diagnosed soft tissue sarcomas (STS). Methods The clinical data of 34 patients with STS undergoing18F-FDG PET-CT before treatment were analyzed retrospectively. The relationship between SUVmax of PET-CT and prognostic factors was evaluated by Mann- Whitney" s non-paraetric test and Spearman" s rank correlation test. The prognostic factors were analyzed by univariate and multivariate analysis. Results Among 34 patients, the median SUVmax was 10.3 (1.5-28.2), and the median maximum diameter was 6.7 cm (1.8-17.2 cm) with positively association between them (r = 0.389, P = 0.028). SUVmax was also associated with pathological grade, AJCC staging and distant metastasis, respectively (all P 〈 0.05). In univariate analysis, distant metastasis, pathological grade, AJCC staging, and SUVmax were found to be the prognostic factors (all P 〈 0.05). Multivariate analysis results indicated that only the SUVmax and distant metastasis were the independent unfavorable prognostic factors (both P 〈 0.05). Conclusions TheSUVmax of pretreatment 18F-FDG PET-CT is well correlated with prognostic factors, and it can predict the prognosis of patients with STS.
