摘 要:目的探讨不同视神经损伤阶段的原发性开角型青光眼(primary open angle glaucoma,POAG)患者的血清细胞因子水平差异及临床价值。方法选取2012年8月至2014年8月我院收治并确诊的POAG患者65例设为观察组,根据视神经损伤程度分为轻度损伤组(A组)、中度损伤组(B组)及重度损伤组(C组),另纳入30例白内障患者作为对照组,对观察组与对照组患者的白细胞介素-2(interleukin-2,IL-2)、IL-4、IL-6及IL-12等血清细胞因子水平进行检测并比较。结果观察组IL-2、IL-4、IL-6及IL-12的水平分别为(90.51±40.61)pg·mL-1、(241.54±78.02)pg·mL-1、(34.84±29.38)pg·mL-1、(94.91±53.89)pg·mL-1,对照组分别为(89.92±38.01)pg·mL-1、(185.01±84.07)pg·mL-1、(62.01±28.05)pg·mL-1、(130.01±81.58)pg·mL-1,观察组IL-6、IL-12水平显著低于对照组,而IL-4水平显著高于对照组,差异均有统计学意义(均为P<0.05),但两组间IL-2差异无统计学意义(P>0.05)。A组IL-12水平显著高于C组(P<0.05),而A、B、C三组IL-2、IL-4、IL-6等水平对比,差异均无统计学意义(均为P>0.05)。结论 IL-12水平在不同视神经损伤阶段的POAG患者存在较大差异,与IL-6可能共同起到对视网膜神经节细胞的保护作用,而IL-4则可损伤视网膜神经节细胞,提示检测血清细胞因子水平可反映视神经损伤的进程。Objeetive To explore the serum cytokines level difference and clini- cal value at different stages of optic nerve injury in primary open-angle glaucoma pa- tients. Methods Sixty-five patients with primary open-angle glaucoma in our hospital from August 2012 to August 2014 were chosen and set as observation group, according to the optic nerve damage degree the patients were divided into mild injury group ( group A), moderate injury group ( group B ) and severe damage group ( group C ), and another 30 patients with cataract were set as control group, the serum levels of cytoldnes, such as interleukin-2 ( IL-2 ), IL-4, IL-6, IL-12, were tested and compared in the observation group and control group. Results The serum levels of IL-2, IL-4, IL-5 and IL-12 in the observation group were (90. 51 ± 40. 51 )pg·mL^-1, (241. 54 ± 78. 02)pg·mL^-1, (34.84 ±29.38)pg·mL^-1, (94.91±53.89)pg·mL^-1 ,respectively,which in the control group were (89.92 ±38.01 )pg·mL^-1, ( 185.01 ± 84.07 )pg·mL^-1, (52.01 ± 28.05 )pg·mL^-1 , (130.01± 81.58)pg·mL^-1 ,respectively,the level of IL-5 and IL-12 in the obser- vation group were obviously lower than those in the control group,but the level of IL4 was higher( all P 〈 0.05 ), but there was no statistical difference in IL-12 between twogroups (P 〉 0.05 ). The level of IL-12 in group A was obviously higher than group C (P 〈 0.05 ), but there was no statistical difference in IL-2, IL-4, IL-6 among group A, B and C ( all P 〉 0.05 ). Conclusion IL-12 level at different phases of optic nerve injury in patients with primary open-angle glaucoma is greatly different, it and IL-6 play a role in protecting retinal ganglion cells, while IL-4 can damage the retinal ganglion cells, which suggest that detection of serum cytokine levels can re- flect the process of optic nerve injury.
