摘　　要：目的 探讨卵黄囊瘤（YST）的影像学特征.方法 回顾性分析经手术病理证实的11例YST影像学表现,7例行CT平扫和增强扫描,4例行MRI平扫和增强扫描.结果 YST位于睾丸4例,隐睾1例,卵巢5例,纵隔1例.4例睾丸、1例隐睾肿瘤CT平扫肿块密度不均匀,2例以囊性成分为主,1例睾丸肿瘤可见腹膜后肿大淋巴结;5例卵巢肿瘤,3例MRI平扫表现为以囊性成分为主的囊实性肿块,其中2例呈“蜂窝状”改变,1例MRI平扫表现为实性成分为主肿块影,1例CT平扫表现为以囊性成分为主的肿块影;1例纵隔肿瘤CT平扫呈不均匀轻度“蜂窝状”低密度肿块影,所有病例增强扫描分隔及实性部分强化,囊性坏死区域未见强化,其中7例呈渐进性强化,5例肿块内见条状强化血管影.结论 YST的影像表现具有一定特征性,结合血清甲胎蛋白（AFP）、年龄等临床资料有助于其诊断和鉴别诊断.Objective To investigate the imaging features of yolk sac tumor. Methods Imaging findings of 11 cases with yolk sac tumors proved by surgery and pathology were analyzed retrospectively. Plain and enhanced CT scans were performed in 7 patients. Plain and enhanced MRI scans were performed in 4 patients. Results 4 cases located in the testicle,1 case in the pelvic cryptorchid, 5 cases in the ovary, 1 case in the mediastinum. On CT scan the density of testicle and pelvic tumors were uneven,cystic component based were showed in 2 cases,and retroperitoneal lymph node enlargenment was seen in 1 case of testicle tumor;For 5 cases ovarian tumors,on MRI scan,cystic component-based solid and cystic mass were showed in 3 cases,in which ＂honeycomb＂ signal intensity was showed in 2 cases,solid mass in the main component was showed in 1 case. The main ingredient for cystic mass was showed in 1 case on CT scan. For 1 case of mediastinal tumor, mild uneven ＂honeycomb＂ low density was showed on CT scan. On enhanced scan, separated enhancement and solid part enhancement were showed in all cases, cystic necrosis was no enhancement, progressive en- hancement in 7 patients, strip vascular enhancement within mass in 5 cases. Conclusion Imaging findings of yolk sac tumor have some certain characteristics,diagnosis and differential diagnosis can be made combining with serum AFP,age and other clinical data.
