摘　　要：目的:观察针灸头部透穴疗法治疗失眠症的临床疗效及作用机制。方法:90例失眠症患者按照随机数字表分为治疗组和对照组,对照组给予常规针刺疗法,治疗组给予头部透穴疗法,采用焦虑自评量表(SAS)和匹兹堡睡眠质量量表(PSQI)评价两组患者治疗前后的焦虑状态和睡眠质量,取两组患者治疗前后静脉血,检测血清5-羟色胺(5-HT)、去甲肾上腺素(NA)和多巴胺(DA)水平,比较两组患者治疗后的效果。结果:治疗后两组患者的SAS和PSQI积分较治疗前均有降低,差异有统计学意义(P<0.05);治疗完成后治疗组的SAS和PSQI评分均低于对照组,差异有统计学意义(P<0.05);治疗完成后治疗组血清5-羟色胺、去甲肾上腺素和多巴胺水平均较治疗前升高,并高于对照组,差异有统计学意义(P<0.05);治疗组的总有效率为91.11%,显著高于对照组的73.33%,差异具有统计学意义(P<0.05)。结论:头部透穴疗法治疗失眠症临床疗效确切,可以改善患者的焦虑状态和睡眠质量,其作用机制与5-羟色胺、去甲肾上腺素和多巴胺水平有关。Objective： To research the mechanism and the efficacy of acupuncture on head therapy for treatment of insomnia. Methods： Ninety insomnia patients were randomly divided into treatment group and control group. The control group used conventional acupuncture therapy,and the treatment group used the head point-through-point therapy. After treatment,SAS and PSQI were used to evaluate the anxiety status and sleep quality,and the levels of 5-HT,NE and DA were tested and compared after treatment. Results： The total effective rate of treatment group was 91. 11%,which was higher than that of control group（ 73. 33%）,differences were statistically significant（ P ﹤ 0. 05）. Before treatment the SAS and PSQI score have statistical significance（ P〉0. 05）; after treatment the scores of SAS and PSQI of treatment group were lower than the control group,differences were statistically significant（ P〈0. 05）. The levels of 5-HT,NE and DA of the treatment group were higher than that of control group,with statistical significance（ P〈0. 05）. Conclusion： The clinical efficacy of head point-through-point therapy for treatment of insomnia is good. It improves sleeping quality and elevates stress level.
