空洞性肺癌与空洞性肺结核的CT鉴别诊断

摘　　要：目的分析空洞性肺癌与空洞性肺结核的CT特征,评价CT对其鉴别诊断的价值。方法回顾分析经病理及临床证实的空洞性肺癌46例,空洞性肺结核46例;分析空洞壁及周围的影像特点,提高对其的诊断和鉴别诊断。结果空洞性肺癌46例,厚壁41例,薄壁5例,其中38例为偏心性,有浅液平7例,呈分叶状42例,伴卫星灶4例,胸膜尾征37例,血管集束征31例,胸腔积液26例,淋巴结肿大22例。空洞性肺结核46例,其中薄壁38例,厚壁8例,其中偏心空洞4例,有壁结节5例,毛刺6例,卫星病灶37例,胸膜尾征5例,纵隔淋巴结肿大4例,病灶临近胸膜下脂肪线增厚19例。结论 CT在空洞性肺癌和空洞性肺结核的鉴别诊断方面有重要价值,提高对肺癌空洞和肺结核空洞CT征象的认识能有效地减少误诊。Objective To analyze the CT features of cavities lung cancer and cavities tuberculosis,and evaluate CT value in differential diagnosis. Methods Retrospectively analyze the cavities lung cancer in 46 cases and the cavities tuberculosis in 46 cases. All patients were confirmed by histopathology and clinical study. CT manifestations of cavity wall and the CT image characteristics of the surrounding were analyzed for cavity lung cancer and tuberculosis empty. Results Of 46 cases with cancer, there were 41 cases of thick cavities, 5 cases of thin cavities, of which there were 38 cases of eccentric, 42 cases of lobular, 4 patients with satellite lesions, 37 cases of pleura] tail sign, 31 cases of vascular convergence sign, 26 cases of pleura] effusion, 22 cases of mediastinal lymph nodes. Of 46 cases with cavity tuberculosis, there were 38 cases of thin cavities, 8 cases of thick cavities, 4 cases of eccentric holes, 6 cases of spiculation, satellite lesions in 37 cases, 5 cases of mural nodule, 4 cases of mediastinal lymph nodes. Conclusion CT has significant value in differential diagnosis of pulmonary cavities of cancer and tuberculosis. It is guarantee to reduce CT misdiagnosis rate by means of improving understanding of basic CT features of cavity tuberculosis and cancer cavity lung.
