耳部轴型皮瓣的临床运用

摘　　要：目的探讨耳部轴型皮瓣在耳廓、耳周皮肤及组织缺损修复中的临床运用价值。方法回顾性分析我院采用含颞浅动脉、耳后动脉的耳前后轴型皮瓣修复因肿瘤、外伤、感染等原因造成的耳廓皮肤、软骨及耳前皮肤组织缺损的16例患者的临床资料,所有病例均采用耳前、耳后轴型皮瓣转移至缺损处修复缺损,行二期断蒂及皮瓣整复术。结果 16例患者转移皮瓣全部存活,断蒂后皮瓣移植区皮片色泽正常、能最大程度维持耳廓的外观形态。结论轴型皮瓣血供好,皮瓣存活率高,尤其适用于修复血循环差的部位如耳廓的缺损,且轴型皮瓣丰富的血供对于防止感染造成菜花耳畸形。Objective To report use of tubed pedicle flap in repairing skin and/or tissue defects of and around the ear.Methods Clinical data were collected from 16 patients whose skin and/or tissue defects of and around the ear from tumor,trauma or infection were repaired by the authors with pre-/post-auricular tubed pedicle flap that contained the superficial temporal or post-auricular artery. The flap was separated from the donor area and reconstruction completed in a second stage procedure. Results The flap survived well after separation form the donor area with normal skin color and helped maintain desired auricular contour in all patients. Conclusion The tubed pedicle flap reported in this study has the advantage of carrying blood circulation which improves flap survival and makes the flap especially suitable for repair of auricular defects that lack adequate blood supply. The abundant circulation inherent to the flap may also help prevent local infection, which is of appreciable value clinically.
