经尿道双极等离子前列腺剜除术对良性前列腺增生尿道功能与性功能影响研究

摘　　要：目的分析经尿道双极等离子前列腺剜除术对良性前列腺增生尿道功能与性功能的影响。方法选择2013-01—2014—12我院收治行良性前列腺增生治疗的患者122例为研究对象，采用随机数字表法分为观察组和对照组各61例。对照组采用开放性前列腺切除术，观察组采用经尿道双极等离子前列腺剜除术，比较两组患者术后的性功能及尿道功能。结果观察组发生勃起功能障碍、逆行射精和尿失禁分别为8．20％、3．28％和1．64％，明显低于对照组的26．23％、18．03％和13．11％，P〈0．05。残余尿量（20．66±10．21）mL，明显少于对照组的（36．47±13．68）mL；最大尿流速率（13．95±2．06）mL／s，明显高于对照组的（10．25±2．16）mL／s，差异有统计学意义，P〈0．05。结论经尿道双极等离子前列腺剜除术有助于改善良性前列腺增生患者的尿道功能，并减少对性功能的影响，值得临床推广应用。OBJECTIVE To analyze the plasma bipolar transurethral enucleation of the prostate for benign prostatic hyperplasia affect urethral function and sexual function. METHODS January 2013 divided into two groups and the con- trol group 61 patients in the control group with open prostateetomy in patients with benign prostatic hyperplasia treated in our hospital 122 cases for the study, using random number table during December 2014 resection, observation group tran- surethral bipolar plasma enucleation of the prostate, sexual function and urinary function after the two groups were com- pared. RESULTS There were erectile dysfunction, retrograde ejaculation, urinary incontinence was significantly lower than the control group [（8.20% vs 26.23%, 3.28% vs 18.03%, 1.64% vs 13.11%） （P〈0.05） ; significantly less than the residual urine volume in the control group （20.66± 10.21 vs 36.47 ±13.68） mL, maximal flow rate was significantly higher （13.95±2.06 vs 10.25±2.16） mL/s, the difference was statistically significant （P〈0.05）. CONCLUSIONS transurethral bipolar plasma enucleation of the prostate in patients with benign prostatic hyperplasia helps improve urinary tract function, and reduce the impact on sexual function, worthy of clinical application.
