分根术在治疗下颌第一磨牙根分叉病变的临床研究

摘　　要：目的探讨分根术在治疗下颌第一磨牙根分叉病变中的临床效果。方法选择42例患者共44颗根分叉病变的下颌第一磨牙,对其残根行根管治疗术后采用分根术将近远中根分开,分别行桩核冠修复,并制作连冠,术后所有患者随访1年。结果 1年后复查,44颗患牙咀嚼功能良好,修复体稳固无松动,X线检查显示牙槽骨无继续吸收或有少量新生,成功率为90.9%。结论在对伴有根分叉病变的下颌第一磨牙的治疗中,残根在经完善的治疗后行分根术保存修复,有利于患牙恢复良好的咀嚼功能,值得临床推广应用。Objective To investigate the clinical effect of split root technique in the treatment of bifurcation lesions of mandibular first molar root. Methods A total of 44 teeth collected from 42 cases of patients with root bifurcation lesion of mandibular first molar reveived the root canal therapy, followed by separation root surgery,pile nuclear crown restoration, and combined crown preparation. All patients were followed up for 1 yearafter surgery. Results 1 year after treatment,44 teeth worked well without mobility. The success rate was 90.9%. Conclusion Residual roots of mandibular first molar with furcation restored with root separation technique can perform masticatory function well and beworthy of clinical application.
