晚期口腔鳞癌术后同期联合放化疗与单纯放疗疗效的比较观察

摘　　要：目的:比较晚期口腔鳞癌术后同期联合放化疗和单纯放疗的临床治疗效果。方法:收集2009年2月至2011年3月我院颌面外科收治的50例晚期口腔鳞癌术后给予同期放化疗治疗的病例,将患者分为联合治疗组(放化疗)20例,对照组(单纯放疗)30例,两组手术及放疗方案相同,比较两组患者的临床疗效、不良反应、生存率。结果:联合治疗组患者的总有效率为80.0%,高于对照组的56.6%(P<0.05)。1年生存率联合治疗组患者为75.0%,对照组为70.0%;2年生存率联合治疗组为65.0%,对照组为36.6%,2年生存率组间比较差异有统计学意义(P<0.05)。联合治疗组患者恶心呕吐和白细胞减少的发生率高于对照组(P<0.05)。结论:晚期口腔鳞癌术后行同期联合放化疗可提高患者的疗效和生存率。Objective：To compare the clinical therapeutic effect of concurrent chemoradiotherapy and radiotherapy alone in postoperative treatment of advanced oral squamous cell carcinomas.Methods：Fifty cases of advanced oral squamous cell carcinomas werevcollected from February,2009 to March,2011 at oral and Maxillofacial Surgery Department of Dental Hospital of Guangxi Medical University.We divided patients into 20 cases of the combination therapy group（chemoradiotherapy）and 30 cases of the control group（radiotherapy alone）.The clinical curative effect,adverse reactions and survival rate of two groups with the same operation and radiation methods were compared.Results：The total effective rate in the combined treatment group（80.0%）was higher than that in the control group（56.6%）（P〈0.05）.The one-year（75.0%）and two-year（65.0%）survival rates of the combination therapy group were higher than those of the control group（one-year survival rate in 70.0%and two-year survival rate in 65.0%）.There was significant difference between two group in two-year survival rate（P〈0.05）.The incident rate of nausea,vomiting and hypoleucocytosis in combined treatment group was higher than those in the control group（P〈0.05）.Conclusion：The postoperative concurrent chemoradiotherapy may improve the curative effect and survival rate of the patients with advanced oral squamous cell carcinomas.
