摘 要:目的探讨Sirt1和ProExC在宫颈高度鳞状上皮内病变(HSIL)细胞学诊断中的价值。方法用免疫细胞化学技术检测436例液基细胞涂片(意义未明非典型鳞状上皮[ASCUS]213例,低级别鳞状上皮内病变[LSIL]101例,高级别鳞状上皮内病变[HSIL]122例)Sirt1和ProEx C表达情况;统计其在诊断宫颈HSIL的敏感度、特异度、准确度等评价指标。结果 (1)Sirt1和ProEx C在预测ASCUS及LSIL中HSIL的敏感度分别为81.48%和74.07%,特异度分别为59.58%和71.43%,准确度分别为61.46%和71.65%,平行试验显示:Sirt1和ProEx C联合应用在预测ASCUS及LSIL中HSIL的敏感度为96.30%,特异度为42.51%。(2)Sirt1和ProEx C在诊断宫颈HSIL的敏感度分别为98.97%和100%,特异度为64%和76%,准确度为91.80%和95.08%,平行试验显示:两者联合应用其敏感度为100%,特异度为48.00%。结论 Sirt1和ProEx C联合检测可提高宫颈HSIL细胞学诊断敏感度,降低漏诊率。Objective To study the Usefulness of Sirtl and ProEx C in the cytologic diagnosis of cervical squamous intraepithelial lesions(SIL). Methods The expression of Sirtl and ProEx C in 436 liquid-based cytologic samples(213 were ASCUS(atypical squamous cells of undetermined significance ), 101 were LSIL(low-grade squamous intraepithelial lesion), 122 were HSIL(high-grade squamous intraepithetial lesion))were done by immunocytochemistry, and their sensitivity,specificity and accuracy in the cytologic diagnosis of cervical HSIL were evaluated. Results (1)The sensitivity,specificity, accuracy of Sirtl and ProEx C in predicting the HSIL in ASCUS and LSIL cytologic results were 81.48% and 74. 07% ,59. 58% and 71. 43% ,61. 46% and 71.65% ,respectively;combined with Sirtl and ProEx C in predicting the HSIL in ASCUS and LSIL cytologic results, the sensitivity was 96. 30% and the specificity was 42.51%. (2)The sensitivity, specificity, accuracy of Sirtl and ProEx C in diagnosis of HSIL were 98.97 % and 100 %, 64% and 76%,91.80 %and 95.08 %, respectively; combined with Sirtl and ProEx C, the sensitivity was 100 % and the specificity was 48.00%. Conclusion Sirtl and ProEx C can improve the sensitivity in the cytologic diagnosis of HSIL.
