摘 要:目的:探讨舌癌患者口底淋巴结的发现及转移情况,以引起手术根治中对口底淋巴结的重视。方法:收集华中科技大学附属同济医院口腔颌面外科,在2012年4月至2014年10月期间,73例经病理活检确诊后实施舌颌颈联合根治的舌癌患者。对术中连续切除的组织,进行各区域淋巴结的筛选并分组,通过病理切片检查各个组的转移情况,并采用卡方检验来处理相关数据,判断影响因素的相关性。结果:本研究的73例舌癌患者中,筛查出24例患者存在口底淋巴结,发现口底淋巴结的发现率为32.9%,其中有4例患者口底淋巴结出现肿瘤的转移,转移率为16.7%。所有患者口底及颈部筛选的淋巴结共计863枚,转移的淋巴结共计89枚,主要集中于I^III区及口底区域,并且口底与I-III区的淋巴结转移率相近(P>0.05)。结论:1SCCT患者的口底组织中可能存在转移的口底淋巴结;2口底淋巴结与I-III区的淋巴结转移率相近。Objective: To determine the value of lingual nodes in combined radical neck dissection for primary squa- mous cell carcinoma of tongue. Method: We reviewed 73 patients with SCCT treated by combined radical neck dissection in the Department of Stomatology, TongJi Hospital of Huazhong University of Science and Technology from Apr 2012 to Oct 2014.By checking the lymph nodes of excision tissues. Reviewing the clinical history and pathological data, we used the Chi-square test to judge the correlation between these factors and lingual nodes metastases. Result: Lingual nodes were found in the floor of mouth of 24 patients.The detection rate of lingual nodes was 32.9 % among 73 patients. We found 863 lymph nodes in all pa- tients, 88 lymph nodes which were located at levels I-III or the floor of mouth were positive.The metastasis rate of lingual nodes was similar to cervical lymph nodes, P 〉 0.05. Conclusion: The possibility of lingual nodes metastasis of squamous cell carci- noma of the tongue is existing in some cancers.The metastasis rate of lingual nodes of tongue cancer in different levels is similar.
