摘 要:目的检测泪腺腺样囊性癌(ACC)组织中血管内皮生长因子(VEGF)和缺氧诱导因子-1α((HIF-1α)的表达,并观察其与泪腺ACC生物学行为之间的相关性。方法实验研究。选取1991年1月至2011年6月在天津医科大学第二医院眼科病理室的60例泪腺ACC患者的手术标本为研究对象,其中腺样一管状型36例,实体型24例。45例为初发瘤,15例为复发瘤(二次或多次复发)。另外选择10例多形性腺瘤患者的瘤旁正常泪腺组织做对照研究。应用免疫组化SP法检测VEGF和HIF-1α的表达。结果泪腺ACC组织中VEGF和HIF-1α的阳性表达率明显高于正常泪腺组(x^2=7.793,P〈0.01;x^2=5.955,P〈0.05),实体型明显高于筛管型(x^2=4.922,P〈0.05;x^2=9.386,P〈0.01),复发瘤明显高于初发瘤(x^2=5.714,P〈0.05;x^2=5.714,P〈0.05)。Spearman相关分析结果显示,VEGF阳性表达率与HIF-1α的阳性表达率具有相关性(rs=0.684,P〈0.05)。结论HIF-1α及和VEGF在泪腺ACC组织中有较高的阳性表达,且与病理类型及复发相关。HIF-1α可能通过影响肿瘤血管的生成使泪腺ACC具有相应的侵袭转移的生物学行为,检测VEGF和HIF-1α的表达有望成为判断泪腺ACC恶性程度、复发与转移行为的生物学指标。Objective To investigate the expression of vascular endothelial growth factor (VEGF) and hypoxia inducible factor-1α(HIF-1α) in adenoid cystic-carcinoma (ACC) of lacrimal gland as well as their relation with biological behavior of adenoid cystic carcinoma. Methods An experimen- tal study. Sixty cases of adenoid cystic carcinoma of lacrimal gland were collected as the research objects. Based on histological revision, there were 36 cases of cribriform-tubular subtype and 24 cas- es of solid subtyper Forty-five cases were primary lesions and 15 cases were recurrent lesions. Ten samples of normal lacrimal gland around polymorphic adenoma were selected as the control group. The expression of VEGF and HIF-1α were evaluated by immunohistochemistry. Results The posi- tive rate of VEGF and HIF-1αin the samples of adenoid cystic carcinoma of lacrimal gland were higher than those in the normal lacrimal gland (x^2=7.793, P 〈0.01; 2,2=5.955, P 〈0.05), the cribri- form-tubular subtypes had higher positive rate of VEGF and HIF-1α than the solid subtypes (x^2=4.922, P 〈0.05; x^2=9.386, P 〈0.01); the recurrent lesions had higher positive rate of VEGF and HIF-1α than the primary lesions (x^2=5.714, P 〈0.05; x^2=5.714, P 〈0.05). The positive rate of VEGF in ACC of lacrimal gland patients was correlated with the positive rate of HIF-1α respectively (rs= 0.684, P 〈0.05). Conclusions The expression of VEGF and HIF-lct are higher level in adenoid cys- tic carcinoma of lacrimal gland and are significantly correlated with pathological type and recur- rence. HIF-lapossibly makes corresponding invasion and metastasis biological behavior in ACC of lacrimal gland by tumor vessel form. Detecting the expression of VEGF and HIF-1α may be be- come biological indexes for malignancy and recurrence of adenoid cystic carcinoma of lacrimal gland.
