白内障超声乳化术后囊袋收缩综合征治疗分析

摘　　要：目的探讨对于超声乳化术后囊袋收缩综合征（capsular contraction syndrome，CCS）患者实施Nd：YAG激光治疗和囊袋松解手术的方法和临床疗效。方法选白2012年3月至2015年1月在南昌大学附属医院白内障科对26例（31只眼）超声乳化术后有症状的CCS患者进行了干预，其中21例（25只眼）进展型CCS行Nd：YAG激光前囊膜切开术，5例（6只眼）重度型CCS行囊袋松解手术，随访3个月，观察术后视力、屈光度、撕囊口形态、IOL位置和并发症情况。结果21例（25只眼）进展型CCS经Nd：YAG激光前囊膜切开术后均有效地阻止了囊袋收缩，未再继续进展；5例（6只眼）重度型CCS行囊袋松解手术后视力提高、远视漂移消除、IOL位置改善、囊袋收缩过程结束。结论Nd：YAG激光前囊膜切开术适用于进展型CCS，囊袋松解术主要用于重度型CCS，及时正确地对收缩的囊袋进行处理，可以避免严重的并发症并恢复良好的视觉效果。Objective To investigate the capsular bag contraction syndrome after phacoemulsifica- tion （capsular contraction syndrome, CCS） treated with Nd：YAG laser therapy and bag of pine and clinical method of operation. Methods Twenty-six cases （31 eyes） after phacoemulsification in pa- tients with symptomatic CCS intervention, in which 21 cases （25 eyes） of progressive CCS, Nd： YAG laser anterior capsulotomy, 5 cases （6 eyes） of severe type CCS bag release operation, fol- low-up of 3 months, observation the postoperative visual acuity, diopter, capsulorhexis opening shape, location and complications of intraocular lens. Results Twenty-one cases （25 eyes）, progres- sive CCS after Nd：YAG laser anterior capsulotomy were effectively prevented the capsular bag con- traction, not to continue the progress; 5 cases （6 eyes） of severe type CCS bag released operation improvement in visual acuity, hyperopia drift elimination, IOL position improve, sac contraction pro- cess ends. Conclusions Nd：YAG laser anterior capsulotomy for progressive CCS, bag lysis is main- ly used for severe CCS, timely pocket on the contraction of the proper treatment, can avoid serious complications and good recovery of visual effects.
