板层角膜移植术后继发青光眼的原因分析

摘　　要：目的探讨板层角膜移植术后继发青光眼的原因及防治方法。方法总结自2011年12月至2013年12月于河北省眼科医院住院的行板层角膜移植患者200例（200只眼）中出现继发性青光眼16例（16只眼）的不同发生原因，针对角膜移植患者不同病种进行分组，其中圆锥角膜患者58例（58只眼），病毒性角膜炎患者46例（46只眼），细菌性角膜炎患者54例（54只眼），角膜营养不良患者15例（15只眼），化学烧伤患者27例（27只眼），手术均采用全厚板层角膜移植术，植片与植床等大或植片大于植床0．25mm，重点观察继发青光眼患者角膜移植术前、术后前房深度的变化及行抗青光眼术前、术后前房深度的变化，并对不同病种做分组观察和分析。结果不同病种行板层角膜移植继发青光眼的机率存在明显不同，术后继发青光眼患者角膜移植术前与术后前房有明显变化，行抗青光眼术前与术后无明显变化。结论板层角膜移植术后继发性青光眼的发生率与患者前房深度、角膜直径大小、不同病种及角膜基质弹性、眼前节炎症反应程度有相关性，临床准确分析病因、给予对应处理可有效控制眼压。Objective To explore the causes and prevention methods for secondary glaucoma af- ter lamellar keratoplasty. Methods Summary of secondary glaucoma in 16 cases （16） occurred from 200 cases of corneal transplant patients from December 2011 to December 2013 in Eye Hospital of Hebei Province Andante layer （200） in the occurrence of different reasons for corneal transplantation group of patients with different diseases, including keratoconus patients 58 cases （58）, 46 patients with viral keratitis （46）, 54 patients with bacterial keratitis （54）, corneal dystrophy patients （15 cas- es, 15 eye）, chemical bums 27 patients （27）, were operated with full thick layer of corneal transplan- tation, graft and planting beds and other large or larger than the planting bed graft 0.25mm, the fo- cus of secondary glaucoma patients observed before keratoplasty, the former anti-glaucoma surgery, postoperative changes in anterior chamber depth changes and postoperative anterior chamber depth, and do different diseases grouped observation and analysis. Results The probability of different dis- eases lamellar keratoplasty distinct presence of secondary glaucoma, there were pre- and postopera- tive anterior corneal graft surgery in patients with glaucoma secondary to significant changes in the front line of anti-glaucoma surgery and postoperative no significant change. Conclusions Lamellar keratoplasty secondary glaucoma incidence and depth, corneal diameter, different diseases and corneal elasticity, anterior segment inflammation in patients with anterior extent relevant, accurate clinical analysis of the cause, effective treatment can be given corresponding control intraocular pressure.
