摘 要:的观察分析视网膜血管瘤临床疗效。方法对2011年6月至2014年12月在河南省立医院眼科就诊的10例视网膜血管瘤患者12只眼,经Quantel medica supra577、532、660nm激光,视网膜冷冻合并巩膜外垫压以及PDT治疗后进行回顾性分析。结果9只眼(Ⅰ期1只眼,Ⅱ期8只眼)经光凝后,视力稳定7只眼,视力提高1只眼,视力下降1只眼。1只眼(Ⅱ期)经光凝联合PDT治疗后,视力稳定无变化;2只眼(Ⅲ期)经视网膜冷冻和巩膜外垫压治疗后,视力稳定1只眼,视力下降1只眼。Ⅰ期11个血管瘤,经光凝后,血管瘤萎缩。Ⅱ期7个血管瘤经光凝后,瘤体均明显萎缩,滋养血管萎缩,渗出减轻。2个疵管瘤经光凝后,瘤体得到控制未发展。1个血管瘤经激光联合PDT治疗后,瘤体萎缩,滋养血管萎缩。Ⅲ期2个血管瘤经视网膜冷冻和巩膜外垫压治疗后,瘤体萎缩,滋养血管萎缩,视网膜下液减少,视网膜平复。结论Ⅰ期、Ⅱ期视网膜血管瘤经激光以及PDT治疗可有效控制瘤体发展,Ⅲ期视网膜血管瘤伴局部视网膜脱离者,经视网膜冷冻合并巩膜外垫压治疗可有效复位视网膜并控制瘤体发展。Objective To investigate the therapeutic effects of retinal angioma. Methods The clinical data of l0 cases (12 eyes) were retrospectively analyzed. The methods of treatment were la- ser photocoatulation (577nm, 532nm, 660nm), photodynamic therapy and trans-scleral cryotherapy. Results There were 9 eyes (1 in stage 1, 8 in stage 2) treated with laser photocoatulation, the visu- al acuity remained unchanged in 7 .eyes, improved in 1 eye, decreased in 1 eye. There was 1 eye in stage 2 treated with laser photocoatulation and photodynamic therapy, the visual acuity remained unchanged. Sclera buckling and trans-scleral cryotherapy was performed on 2 eyes, and the visual acuity remained unchanged in 1 eye, decreased in 1 eye. There were 11 retinal angiomas in stage 1 treated with laser photocoatulation, and all of the angiomas were atrophied after the treatment. There were 7 retinal angiomas in stage 2 were atrophied obviously after the treatment of laser photocoatula- tion, and attenuation of the afferent vessels, reduction of intraretinal exudate with laser photocoatula- tion. Two retinal angiomas were controlled after the treatment. One retinal angioma was atrophied af- ter the treatment of laser photocoatulation and photodynamic therapy, and attenuation of the afferent vessels. There were 2 retinal angiomas in stage 3 were atrophied, the afferent vessels attenuated and the subretinal fluid occurred after the treatment of sclera buckling and trans-scleral cryotherapy. Conclusions Laser photocoagulation and photodynamic therapy are effective in treating the andiomas from stage 1 to 2. The andiomas accompanied by retinal detachment in stage 3 is more favorable in trans-scleral cryotherapy and sclera buckling.
