肾上腺外副神经节瘤15例临床病理分析

摘　　要：目的探讨肾上腺外副神经节瘤临床及病理学特点、良恶性判断及鉴别诊断。方法对15例肾上腺外副神经节瘤病例的临床表现、组织形态学特征及免疫组化特点进行分析。结果肾上腺外副神经节瘤15例,男性4例、女性11例,年龄34~67岁,平均年龄54.9岁。肿瘤位于腹膜后6例,头颈部6例,纵隔、胆囊、腰椎管内各1例。有功能的3例,无功能的12例;组织学上肿瘤主要由主细胞和支持细胞两种细胞组成,免疫组化：肿瘤细胞Cg A、Syn、NSE、vimentin及S-100均（＋）,CK、EMA（-）。结论肾上腺外副神经节瘤的诊断依据组织形态学特点与免疫组化特征性表达。Objective To describe the clinicopathologic features, benign or malignant histological features and differential diagnosis of extra-adrenal paraganglioma. Methods We performed an analysis of 15 cases of extra-adrenal paraganglioma. Clinical manifestations,pathological morphology and immunohistochemical staining were analyzed.Results There were 4 male and 11 female patients aged from 34 to 67 years（ mean 54. 9 years）. Six tumors occurred in the retroperitoneum,six in the neck,and one case in the mediastinum,gall bladder and lumbar vertebra,respectively.There were 3 cases of functional and 12 cases of non-functional paragangliomas. Histologically,the tumor was composed of chief cells and sustentacular cells. Immunohistochemical staining revealed chief cells were intensely positive for neuroendocrine markers such as Cg A,Syn,NSE and VIM,but negative for CK and EMA. Sustentacular cells were positive for S-100 protein. Conclusion Pathological and immunohistochemical features are the key to diagnosis of extra-adrenal paraganglioma.
