溃疡油防治鼻咽癌急性放射性皮炎38例临床观察

摘　　要：目的观察溃疡油防治鼻咽癌患者急性放射性皮炎的临床疗效。方法将74例鼻咽癌放射治疗患者随机分为治疗组38例和对照组36例。两组患者均采用三维适形调强技术放疗,第一阶段放射剂量为56-60 Gy,缩野加量至鼻咽肿瘤病灶处66-76 Gy,中位剂量70 Gy,每周一至周五进行放疗,共放疗33-38次。第1次放射治疗后开始治疗组与对照组患者分别给予溃疡油或三乙醇胺乳膏均匀涂在照射野皮肤上并超出1 cm左右的范围,厚约1-2 mm,放疗后3 h、6 h各涂抹1次。每周采用急性放射性皮炎反应评估表（RISRAS）和美国肿瘤放射治疗协作组（RTOG）评分标准对放射野皮肤损伤进行分级,并记录不良反应。每例患者共观察6周。结果 74例患者均顺利完成放射治疗。两组患者急性放射性皮炎RISRAS评分均随放射剂量的增加而增加,第3周开始治疗组患者RISRAS评分明显低于对照组（P〈0.05）。治疗至第6周时,治疗组Ⅱ级及以上放射性皮炎的发生率13.1%,显著低于对照组的33.3%（P〈0.05）。结论溃疡油能有效防治鼻咽癌患者照射野皮肤放射性损伤,能减轻急性放射性皮炎的症状。Objective To observe the clinical effect of Kuiyangyou preventing and treating nasopharyngeal carcinoma acute radiodermatitis. Methods Seventy-four nasopharyngeal carcinoma patients by radiotherapy were randomized into the treatment group（ 38 cases） and the control group（ 36 cases）. Both groups were given radiotherapy adopting three-dimensional conformal adjusting technology. On the first stage,the radiation dose was 56- 60 Gy,reduced field boost to 66- 76 Gy at nasopharyngeal neoplasms lesion,median dose 70 Gy. Give radiotherapy from Monday to Friday,33- 38 times in total. After the first time of radiotherapy,patients in the treatment group and the control group applied Kuiyangyou or trolamine cream respectively to radiation field skin evenly and out of 1 cm range,thickness of about 1- 2 mm. Apply once 3 h and 6 h after radiotherapy. Grade the radiation field skin adopting The Radiation-Induced Skin-Reaction-Assessment-Scale（ RISRAS） and radiation therapy oncology group（ RTOG） scoring standard. Record the side effects. Observe each patient for 6 weeks. Results All patients completed radiotherapy successfully. The RISRAS scores in both groups increased with the rise of the radio dose. From the third week,RISRAS score in the treatment group was obviously lower than that in the control group（ P〈 0. 05）. When treating for the sixth week,the incidence of radiodermatitis of Ⅱ and more grades in the treatment group was 13. 1%,obviously lower than 33. 3% in the control group（ P〈 0. 05）. Conclusion Kuiyangyou could prevent and treat radiation damage of nasopharyngeal carcinoma patients ＇ radiation field skin effectively and release the symptoms of acute radiodermatitis.
