摘  要:目的检测内皮素-3(EDN-3)基因启动子甲基化及其表达在乳腺癌组织中的变化,并分析其临床意义。方法采用甲基化特异PCR(MS-PCR)和实时定量PCR(RT-PCR)方法检测42例乳腺癌组织(实验组)及50例正常乳腺组织(对照组)EDN-3基因启动子甲基化发生率及其mRNA表达,比较以上两指标变化在乳腺癌各临床参数之间的统计学差异。结果实验组EDN-3启动子甲基化检出率为64.29%(27/42),对照组EDN-3启动子甲基化检出率为8.00%(4/50),差异有统计学意义(2=42.17,P<0.01),该检出率在患者不同年龄,不同肿瘤组织类型和大小,有否区域淋巴结转移及远处转移之间差异均无统计学意义(P>0.05)。实验组EDN-3 mRNA表达较对照组显著降低,差异有统计学意义(t=-8.21,P<0.01),但这种降低在上述不同临床参数之间差异亦无统计学意义(P>0.05)。结论 EDN-3基因异常甲基化与乳腺癌密切相关,它能影响其m NRA转录水平,对其进行检测能为乳腺癌的早期诊断和判断预后提供帮助。Objective To investigate the mRNA expression and methylation of endothelin- 3 (EDN -3 ) promoter in patients with breast cancer and to evaluate the clinical significance. Methods The methylation rates and mRNA expressions of EDN - 3 gene in 54 patients with breast cancer ( experimental group) and 60 healthy women (control group) were assessed with methylationspecific polymerase chain reaction (MS- PCR) and, real -time quantitative polymerase chain reaction ( RT - PCR) respectively. Statistical differences of the two index changes in the clinical parameters of breast cancer were compared. Results The detection rate of methylation of EDN - 3 pro- moter was 55.56% (30/54) in experimental group and 8.00% (4/5) in the control group, there were significant differences between those two groups ( χ2 =42. 17, P 〈0. 01 ). However there were no statistical differences in terms of the age, tumor size and type, regional lymph node metastasis and distant metastasis ( P 〉 0.05 ). There were significant differences between the two groups in terms of the expression of endotbelin - 3 mRNA which significantly decreased in breast cancer patients compared to that of healthy women ( t = - 8.21, P 〈 0.01 ). But there were no statistical differences among all the clinical parameters ( P 〉 0. 05 ). Conclusion Abnormal methylation of EDN - 3 promoter is closely correlated to the occurrence and development of breast cancer and could down - regulate the tran- scription of mRNA of EDN - 3, thus providing biomarker for early diagnosis and prognosis of breast cancer.
