简化的引导性骨组织再生术在上前牙区骨量不足增量术中的应用

摘　　要：目的 探讨简化的GBR在上前牙区牙槽骨宽度不足的应用效果,并对使用和不使用胶原生物膜的成骨效果进行比较。方法 选取广东省口腔医院2012年2月~2014年1月期间,因上前牙区缺牙后导致牙槽骨宽度不足的36例患者（共82个缺牙位点）,将之随机分为两组：A组：植入人工骨粉后覆盖胶原膜的19例;B组：植入人工骨粉而不盖胶原膜的为17例。两组均于GBR后6个月行一期种植手术,分别植入植体43颗和39颗。采用锥形束CT测量两组患者在种植前、GBR 6个月后唇腭向牙槽嵴的宽度变化,并进行统计学分析。结果 GBR 6个月后,盖膜组与未盖膜组的骨宽度等得到较好的增加,两者的骨宽度变化均无统计学差异（P〉0.05）。结论 简化的GBR应用于上前牙区牙槽骨宽度不足病例,可获得较好的临床效果,同期盖膜相对于不盖膜,对骨增量情况无影响。Objective To investigate the application effect of GBR （guided bone regeneration） in anterior maxillary region with lack of width of alvelor bone, and to compare the difference of osteogenesis with or without the use of collagen biological membrane. Methods 36 patients were selected who accept treatment in Stomatological Hospital of Guangdong Province for lack of width of alvelor bone in anterior maxillary region from February 2012 to January 2014 and randomly assigned into two groups. Group A： 19 patients who were covered with collagen biological membrane after filled wtih alloplastic bone substitutes; Group B： 17 patients who were filled with alloplastic bone substitutes without coverage of collagen biological membrane. Both groups had taken one period of implant surgery in 6 months after GBR, separately implanted 43 and 39 implants of Ankylos system. Cone beam CT was adopted to measure the labial - palatal alveolar ridge width of both groups before the implantation and 6 months after load. Statis- tical analysis was processed. Results There were no statistical differences between these two groups in terms of the labial- palatal alveolar ridge width while their widths all have positive augmentations. Conclusion Good clinical effect could be obtained by the application of simplified GBR in the patients with lack of width of alvelor bone in anterior maxillary region, there is no difference in terms of the alvelor bone width augmentation with or without collagen biological membrane.
