摘 要:目的:探究与分析24小时动态脑电图在小儿癫痫诊断中的临床应用价值。方法选取2014年2月至2014年10月徐州市儿童医院小儿癫痫住院患儿122例和同期非小儿癫痫住院患儿122例,分别应用24小时动态脑电图和常规脑电图进行诊断检查。统计癫痫住院患者的睡眠分期状况,同时对比24小时动态脑电图和常规脑电图对住院癫痫患儿的检出率,住院癫痫患儿和住院非癫痫患儿的痫样放电率。结果小儿癫痫住院患者发作期在觉醒期、睡眠期以及觉醒期和睡眠期的比例分别是:9.6%、47.2%、43.2%;24小时动态脑电图小儿癫痫检出率是80.33%,常规脑电图小儿癫痫检出率是28.69%,两组检出率相比具有统计学差异(χ^2=12.55,P<0.05);癫痫患儿组痫样放电率67.2%,非癫痫组患儿痫性放电率是8.2%,两组痫性放电率相比具有显著统计学差异(χ^2=17.21,P<0.05)。结论24小时动态脑电图对小儿癫痫诊断的正确率较常规脑电图有明显提高,优势明显,可作为小儿癫痫诊断的理想方法,值得在临床推广应用。Objective To explore and analyze the application value of 24-hour ambulatory EEG in diagnosis of epilepsy in children. Methods From February to October in 2014 122 cases of pediatric epilepsy and 122 cases without pediatric epilepsy admitted in Children’ s Hospital of Xuzhou City were selected at same period, and 24-hour ambulatory EEG and routine EEG were applied for diagnosis, respectively.Sleep stages of hospitalized patients with epilepsy were recorded, and 24-hour ambulatory EEG was compared with routine EEG in detection rate of hospitalized children with epilepsy and epileptiform discharge rate between epilepsy and non-epileptic children. Results The proportions of epilepsy attach of inpatients in waking, sleeping, and waking and sleeping period were 9.6%, 47.2% and 43.2%, respectively.Epilepsy detection rate of 24-hour ambulatory EEG was 80.33%, and that of conventional EEG was 28.69%.The detection rate of two groups was significantly different (χ^2 =12.55, P〈0.05).The epileptiform discharge was 67.2% and 8.2% in epilepsy group and non-epileptic group, respectively, and the difference was significant(χ^2 =17.21, P〈0.05).Conclusion The 24-hour ambulatory EEG obtains significantly higher diagnostic accuracy for epilepsy in children than conventional EEG.It has obvious advantages and can be used as an ideal method in diagnosing epilepsy in children, which is worthy of clinical application.
