益气活血方对消化性溃疡愈合质量及胃黏膜组织中COX-1、COX-2和PGE_2表达的影响

摘　　要：目的观察益气活血方对消化性溃疡溃疡愈合质量的影响及对溃疡黏膜组织中环氧合酶(COX)-1、COX-2和前列腺素E2(PGE2)的调节作用,探讨COX-1、COX-2和PGE2的表达水平与溃疡愈合质量的关系。方法根据随机对照原则将60例消化性溃疡患者分为中西医组和西医组,每组30例。中西医组给予益气活血方汤剂+奥美拉唑口服,西医组仅给予奥美拉唑口服,2组均以4周为1个疗程。2组中幽门螺杆菌阳性者均给予三联10d根除法治疗。治疗前后均行胃镜及常规病理检查,并取溃疡黏膜组织行COX-1、COX-2、PGE2免疫组织化学染色检测;观察比较2组临床疗效以及溃疡愈合质量。结果 2组临床疗效比较差异无统计学意义。中西医组内镜下再生黏膜成熟度Sc+Sb数量及再生黏膜组织学成熟度优良率有高于西医组的趋势,但差异无统计学意义。2组治疗后溃疡黏膜组织中COX-1、COX-2、PGE2表达水平均较治疗前明显升高(P均<0.05),且中西医组COX-1、COX-2、PGE2表达水平明显高于西医组(P均<0.05)。相关性分析显示,溃疡黏膜组织中COX-1、COX-2、PGE2表达水平与溃疡愈合质量呈正相关。结论益气活血方可能通过增加溃疡黏膜组织中COX-1、COX-2的表达和PGE2的分泌促进溃疡的愈合,提高溃疡愈合质量。Objective It is to observe the effect of Yiqi Huoxue decoction on the healing quality and regulation on COX -1, COX-2 and PGE2 in the treatment for peptic ulcer , and to explore the relationship of ulcer healing quality with the expres-sion of COX-1, COX-2 and PGE2 .Methods According to randomized controlled principle , 60 patients with peptic ulcer were divided into traditional Chinese medicine and western medicine treatment group and western medicine treatment group , 30 cases in each group .Traditional Chinese and western medicine group were treated with oral decoction of Yiqi -huoxue formula plus antacids omeprazole;with omeprazole to western medicine treatment group only .4 weeks （ 28 days ） was one course of treatment in both groups .The triple 10 days Eradication Act would be applied when met patients with Hp -positive peptic ul-cer.Routine endoscopy and biopsy would be performed before and after the treatment , and the levels of COX -1, COX-2 and PGE2 in ulcer tissue were determined by immunohistochemical staining; the clinical efficacy of treatment and quality as-sessment of ulcer healing were compared between the two groups .Results There was no significant difference in efficacy be-tween the two groups .The endoscopy regeneration mucosal maturity Sc ＋Sb quantity , the good rate of regenerative mucosa his-tological maturity of integrative medicine group had a increase trend compared with that in western group , but the differences were not significant.After treatment, the expression levels of COX -1, COX-1, PGE2 were all increased obviously compared with that before treatment in both group （P all〈0.05）, and the levels in integrative medicine group were higher than that in western medicine group （P all〈0.05）.Correlation analysis showed that the manifestation of gastric mucosa COX -1, COX-2, PGE2 were positively correlated with ulcer healing quality .Conclusion Yiqi-huoxue Formula may promote ulcer healing and improve the quality of ulcer healing by increasing COX -1, COX -2 manifestatio
