哮喘患者体内YKL-40的表达与IL-4、IgE的相关性分析

摘　　要：目的探讨哮喘患者体内血清几丁质酶样蛋白人类软骨糖蛋白40(YKL-40)的表达与白细胞介素4(IL-4)、Ig E的相关性。方法将2013年2月至2014年2月德阳市人民医院收治的120例支气管哮喘患者根据病情分为轻度组(48例)、中度组(42例)和重度组(30例),并选取40例健康体检者作为对照组。测定4组受试者血清YKL-40、IL-4、Ig E水平,并分析YKL-40的表达与IL-4、Ig E的相关性。结果哮喘患者YKL-40、IL-4、Ig E水平均显著高于对照组(P<0.05);哮喘患者血清YKL-40、IL-4、Ig E水平均显著高于对照组(P<0.05);在YKL-40、Ig E水平方面中度组及重度组均显著高于轻度组(P<0.05),重度组显著高于中度组(P<0.05);重度组IL-4水平显著高于轻度组(P<0.05);以YKL-40为因变量,以第一秒用力呼气容积占预计值百分比(FEV1%)、IL-4、Ig E为自变量,YKL-40与IL-4无线性相关关系(P>0.05),YKL-40与Ig E呈线性相关关系(P<0.05),回归方程为y=0.07x+23.73,FEV1%与YKL-40呈线性相关关系(P<0.05),回归方程为y=-0.52x+74.16。结论哮喘患者体内YKL-40表达显著高于正常人群,与患者的病情、Ig E水平、炎症反应均有密切关系,可以作为哮喘诊断、病情评估的相关指标。Objective To investigate the correlation between the expression of YKL-40 and IL-4, IgE in serum of asthma patients. Methods A total of 120 bronchial asthma patients admitted to Deyang City People＇s Hospital from Feb. 2013 to Feb. 2014 were divided into mild group（48 cases） ,moderate group（42 cases） and severe group（ 30 cases） according to the condition of disease, and 40 cases of normal people were included as the control group. Serum YKL-40, IL-4, IgE levels of the four groups were measured and correla- tion between YKL-40 expression and IL-4,IgE were analyzed. Results YKL-40, IL-4, IgE levels were sig- nificantly higher in asthma patients（ P 〈 0. 05 ） ;serum YKL-40, IL-4 and IgE levels of the asthma patients were significantly higher than the control group （ P 〈 0. 05 ） ;level of YKL-40 and IgE in the moderate group and severe group were significantly higher than that in the mild group（P 〈0. 05） ,and the severe group was significantly higher than the moderate group （ P 〈 0. 05 ） ; IL-4 level in the severe group was significantly higher than the mild group（P 〈 0. 05 ） ; when choosing YKL-40 as the dependent variable, FEV1%, IL-4, IgE as independent variables,there was no linear correlation between YKL-40 and IL-4 （ P 〉 0. 05 ）, YKL-40 and IgE showed a linear correlation （ P 〈 0. 05 ）, the regression equation was y = O. 07x ＋ 23.73 ; YKL-40 and FEV1% showed linear correlation （P 〈 0. 05 ）, the regression equation was y = -0.52x ＋ 74. 16. Conclusion YKL-40 expression in asthmatic patients was significantly higher than the normal population, and is close- ly related the patient＇s condition,IgE level,and inflammation,which can he used as indicators of asthma-re- lated diagnosis and assessment.
