摘  要:目的探讨PD伴抑郁与脑组织感兴趣区微观结构变化的相关性。方法根据DSM-IV将40例PD患者分成PD伴抑郁组和PD非抑郁组;对2组进行DTI检查,测定感兴趣区的FA值,对PD伴抑郁组患者行汉密尔顿抑郁量表评估。结果 PD伴抑郁组额叶白质FA值(0.307±0.044)、扣带回前部白质FA值(0.297±0.02)较PD非抑郁组额叶白质FA值(0.370±0.31)、扣带回前部白质FA值(0.324±0.02)明显下降,(P<0.05),其余各ROI的FA值无明显差异(P>0.05)。PD伴抑郁与额叶白质及扣带回前部白质FA值有关(OR=0.00,OR=0.00,P<0.05)。PD伴抑郁组患者额叶白质、扣带回前部白质FA值与HAMD量表得分呈负相关(r=-0.615,r=-0.515,P<0.05)结论 PD伴抑郁患者额叶及扣带回前部白质微观结构存在变化,且此种变化的严重程度与抑郁程度有关。Objective To investigate the relation between depression in Parkinson's Disease( PD) and the microstructure changes of brain regions of interest( ROIs) in diffusion tensor imaging( DTI). Methods Depending on DSM-IV,40 PD patients were divided into DPD( Depression of Parkinson's disease) group and PDND( Parkinson's disease without depression) group. Both groups were measured by DTI to get the fractional anisotropy( FA) values of ROIs. DPD group were studied by Hamilton depression scale( HAMD) to assess the degree of depression. Results The PD group showed significant FA reduction in FWM,ACLB( 0. 307 ± 0. 044,0. 297 ± 0. 02 VS 0. 370 ± 0. 31,0. 324 ± 0. 02; P〈0. 05). In the Logistic Regression analysis,FA in FWM and ACLB show significant correlation to DPD( OR = 0. 00,OR = 0. 00,P〈0. 05). The scores of HAMD in DPD group was negatively correlated with the FA values of FWM and ACLB( r =- 0. 615,r =- 0. 515,P〈0. 05). Conclusions The microstructural damage in FWM and ACLB is positively correlated with cognitive impairment in PD patients. The FA reduction of FWN and ACLB has the potential to serve as a noninvasive early biomarker for DPD.
