临床药师对1例晚期乳腺癌患者疼痛治疗的药学监护体会

摘　　要：目的:探讨临床药师参与晚期癌痛患者止痛治疗的作用。方法:结合1例晚期癌痛患者具体病例,在患者住院全过程从癌痛的全面评估方法、阿片类药物快速滴定的选药原则、维持治疗原则、药品不良反应防范等方面为患者提供药学服务并进行药学监护。结果:本次药学监护在保障患者用药安全、提高患者生存质量、降低患者药品费用、提高患者用药依从性等方面取得明显效果,提高了临床药物治疗水平。结论:临床药师深入临床直接面向晚期癌痛患者提供药学服务,开展药学监护,可在药物选择、防范严重药品不良反应发生等多个方面保障患者用药安全,体现临床药师价值。Objective： To explore the role of clinical pharmacists in pain management of advanced cancer.Methods： Based on one case with advanced cancer pain, clinical pharmacist provided pharmaceutical care for the patient including comprehensive assessment on cancer pain, drug selection for rapid titration of opioid drugs, maintenance treatment principle, and prevention of adverse drug reactions. Results： The pharmaceutical care ensured patient safety, improved the quality of life, reduced drug costs, and improved patient medication compliance to achieve signiifcant effect and improved pharmacotherapy level. Conclusions： Clinical pharmacists guaranteed the safety of drug use and showed their value through providing pharmaceutical care for patients with cancer pain directly, developing pharmaceutical monitoring, selecting drugs rationally, avoiding drug interactions and preventing serious adverse drug reactions.
