高清放大胃镜联合i-scan电子染色诊断早期胃癌的价值

摘　　要：目的:探讨高清放大胃镜联合i-scan电子染色对早期胃癌的诊断价值.方法:选取无锡市锡山人民医院分院2011-01/2013-12 70例疑似胃癌患者作为研究对象,并对其进行高清放大胃镜联合i-scan电子染色检查,并且以手术病理检查作为金标准,评价高清放大胃镜联合i-scan电子染色对早期胃癌的诊断效果.结果:病理诊断检查良性病灶50例,恶性病灶20例.高清放大胃镜联合i-scan电子染色检查良性病灶48例,恶性病灶22例.高清放大胃镜联合i-scan电子染色检查敏感度与特异度分别为95.0%、94.0%.高清放大胃镜联合i-scan电子染色检查与病理诊断结果一致性,Kappa=0.825.20例早期胃癌患者中8例内镜黏膜下剥离术,12例胃癌根治术.高清放大胃镜联合i-scan电子染色评估浸润深度与病理结果一致性达到90.0%,且Kappa=0.981.结论:临床中对于早期胃癌应用高清放大胃镜联合i-scan电子染色诊断具有较好的诊断效果.同时,高清放大胃镜联合i-scan电子染色诊断的敏感度和特异度也比较高,且临床应用价值高.AIM：To evaluate the diagnostic value of highdefinition magnifying endoscopy（HD-ME）combined with i-scan electronic staining in early gastric cancer.METHODS：Seventy patients with suspected gastric cancer treated at Wuxi City People＇s Hospital from January 2011 to December 2013 were included.All patients underwent HDME combined with i-scan electronic staining.Using surgical biopsy as the gold standard,the diagnostic performance of HD-ME combined with i-scan electronic staining was evaluated.RESULTS：Pathological analysis revealed50 cases of benign lesions and 20 cases of malignant lesions.By HD-ME combined with i-scan electronic staining,48 cases were diagnosed to be benign lesions and 22 cases to be malignant lesions.The sensitivity and specificity of HD-ME combined with i-scan electronic staining were 95.0%and 94.0%,respectively,and there was a good consistency between HD-ME combined with i-scan electron staining and histologic diagnosis（Kappa =0.825）.Of 20 patients with early gastric cancer,8underwent endoscopic submucosal dissection,and 12 were treated by surgery.The consistency between HD-ME combined with i-scan electron staining and histology in assessing the depth of invasion was 90.0%（Kappa = 0.981）.CONCLUSION：HD-ME combined with i-scan electron staining has good diagnostic performance and relatively high diagnostic sensitivity and specificity in early gastric cancer.
