EphA2表达水平与食管鳞癌临床病理特征关系研究

摘　　要：目的分析不同临床病理特征食管鳞癌患者Eph A2表达水平的差异,探讨血清中Eph A2表达水平对食管鳞癌临床病理特征的评估价值。方法收集2010—2012年新疆医科大学附属肿瘤医院胸外科行手术治疗的食管鳞癌患者100例为研究对象。取得患者手术切除癌组织标本100例,同时取得远隔癌旁组织标本100例。食管鳞癌患者血清标本由新疆肿瘤防治研究所标本库提供,同时选择新疆医科大学附属肿瘤医院体检科的体检健康者血清60份。采用反转录聚合酶链式反应(RT-PCR)技术检测Eph A2 mRNA表达水平、Western blotting法检测Eph A2表达水平、酶联免疫吸附试验(ELISA)法检测血清中Eph A2表达水平。采用SPSS 16.0统计学软件绘制受试者工作特征曲线(ROC曲线)并计算曲线下面积。结果癌组织中Eph A2 mRNA表达水平高于远隔癌旁组织(t=17.964,P<0.05)。癌组织中Eph A2表达水平高于远隔癌旁组织(t=12.100,P<0.05)。食管鳞癌患者血清中Eph A2表达水平高于体检健康者(t'=18.240,P<0.05)。Ⅰ期+ⅡA期食管鳞癌患者癌组织Eph A2 mRNA表达水平、Eph A2表达水平及血清中Eph A2表达水平均低于ⅡB期+Ⅲ期食管鳞癌患者(P<0.05)。有淋巴结转移食管鳞癌患者癌组织Eph A2 mRNA表达水平、Eph A2表达水平高于无淋巴结转移食管鳞癌患者(P<0.05)。T1期+T2期食管鳞癌患者癌组织Eph A2 mRNA表达水平、Eph A2表达水平及血清中Eph A2表达水平均低于T3期+T4期食管鳞癌患者(P<0.05)。血清中Eph A2表达水平评估食管鳞癌TNM分期的ROC曲线下面积为0.646,95%CI(0.532,0.761),最佳截断值为50.17 ng/L,灵敏度为69.0%,特异度为61.0%;血清中Eph A2表达水平评估食管鳞癌浸润深度的ROC曲线下面积为0.650,95%CI(0.528,0.772),最佳截断值为53.93 ng/L,灵敏度为73.0%,特异度为57.0%。结论 Eph A2表达水平与食管鳞癌的发生发展及转移浸润有关,其在血清中的表达水平对术前疾病分期及浸润�Objective To investigate the difference of Eph A2 expression among esophageal squamous cell carcinoma（ ESCC） patients with different clinic pathological features and to explore the value of the serum expression of Eph A2 for ESCC evaluation. Methods Enrolled 100 ESCC patients who received thoracic surgery in the Affiliated Tumor Hospital of Xinjiang Medical University from 2010 to 2012. Obtained 100 cancer tissue samples excised from the patients, and 100 tissue samples remote to carcinoma were also taken as controls. The serum samples of patients with esophageal squamous cell carcinomas were provided by the specimen repository of Xinjiang Institute of Cacer Prevention and Control,and 60 serum samples of healthy people who received physical examination in the Affiliated Tumor Hospital of Xinjiang Medical University were taken as controls. The mRNA expression level of Eph A2 was measured by RT-PCR, the expression of Eph A2 was measured by Western blotting method,and the expression of Eph A2 in serum was measured by ELISA. SPSS 16. 0 was employed to make ROC curves,and AUC was calculated. Results The mRNA expression level of Eph A2 in cancer tissue was higher than that in tissue remote to carcinoma（ t = 17. 964,P〈0. 05）. The expression level of Eph A2 in cancer tissue was higher than that in tissue remote tocarcinoma（ t = 12. 100, P〈0. 05）. ESCC patients were higher than healthy people in the serum expression of Eph A2（ t＇= 18. 240,P〈0. 05）. Patients at stageⅠand stageⅡA were lower（ P〈0. 05） than patients at stage Ⅱ B and stage Ⅲ in the mRNA expression of Eph A2,Eph A2 expression and Eph A2 expression in serum. Patients with lymphatic metastasis were higher（ P〈0. 05） than patients without lymphatic metastasis in the mRNA expression of Eph A2 and Eph A2 expression.
