早产儿视网膜病变阈值前1型激光光凝的疗效观察

摘　　要：目的 评价810 nm激光治疗早产儿视网膜病变阈值前1型的疗效.方法 回顾性研究.收集2008年1月至2012年12月在北京大学人民医院眼科首次进行激光治疗的早产儿视网膜病变阈值前1型患儿226例（384只眼）的临床资料.所有患儿均进行810 nm激光视网膜光凝,分析激光治疗的疗效和并发症.对Ⅰ区和Ⅱ区病变的消退率比较采用Fisher检验.结果 226例患儿中,男性146例,女性80例,平均出生孕周（30.2±2.08）周,出生体重（1 386±340）g.在所有接受治疗的384只眼中,Ⅰ区病变35只眼,Ⅱ区病变349只眼.治疗后平均随访（15.1±12.4）个月.384只眼中,病变完全消退381只眼,占99.1％;病变消退但残存颞侧增殖膜2例,占0.6％;病变进展4A期发生视网膜脱离1只眼,占0.3％;玻璃体切除术后黄斑在位.Ⅰ区病变激光后病变完全消退率（91.4％）与Ⅱ区病变激光后完全消退率（100.0％）差异有统计学意义（P=0.001）.未发现与治疗相关的严重局部与全身并发症.结论 810 nm激光光凝治疗早产儿视网膜病变阈值前病变1型安全有效.Objective To evaluate diode laser photocoagulation effects in type 1 prethreshold retinopathy of prematurity （ROP） in Peking University People＇s Hospital.Methods Retrospective crosssectional study.Clinical records of the premature infants treated for type 1 prethreshold ROP from the year of 2008 to 2012 were reviewed.Results We reviewed 226 infants including 146 male and 80 female.The mean birth weight and the gestational age were 1 386 grams and 30.2 weeks respectively.In all the 384 photocoagulation treated eyes, ROP regressed completely in 381 eyes （99.1%）.One eye of zone Ⅰ ROP progressed to stage 4A.Macula kept attaching after vitrectomy.There was no significant difference of regression rate between Zone Ⅰ and Zone Ⅱ of type 1 prethreshold ROP.No severe complications occurred.Conclusions Diode laser photocoagulation in type 1 ROP was safe and efficient.
