摘 要:目的探讨人附睾蛋白(HE4)、膜联蛋白A3(Annexin A3)、间皮素(Mesothelin)在上皮性卵巢癌(EOC)血清与组织中的表达及其对卵巢癌铂类耐药的影响。方法选取2011年9月-2014年3月该院妇科收治的78例EOC为研究对象。按铂类药物疗效分为铂类敏感组(62例),及铂类耐药组(16例)。ELISA法检测78例EOC患者血清HE4、Annexin A3、Mesothelin及CA125水平,免疫组化法检测组织中HE4、Annexin A3和Mesothelin蛋白的表达,分析HE4、Annexin A3和Mesothelin与卵巢癌铂类耐药的相关性。结果 78例患者中两组间血清HE4、Annexin A3、Mesothelin、CA125浓度差异均有统计学意义(P<0.05);血清HE4、Annexin A3和CA125阳性表达率差异有统计学意义(P<0.05),血清中4种蛋白的阳性表达两两比较均呈正相关(P<0.05),HE4、Annexin A3、Mesothelin在EOC组织中的阳性表达率差异均有统计学意义(P<0.05),78例组织中HE4、Annexin A3、Mesothelin阳性表达两两比较,差异均有统计学意义(P<0.05);HE4、Annexin A3、Mesothelin在血清和组织中的阳性表达均呈正相关(P<0.05)。结论 HE4、Annexin A3和Mesothelin参与了卵巢癌发生发展,并与铂类耐药密切相关;联合检测其在血清和组织中的表达有助了解卵巢癌铂类耐药的可能机制,预测化疗敏感性,并可为铂类耐药性卵巢癌的治疗提供靶点。Objective To explore the expressions of human epididymis protein 4 ( HE4), annexin A3, and mesothelin in serum and epithelial ovarian cancer (EOC) tissue and their impacts on platinum resistance of ovarian cancer. Methods Seventy-eight EOC patients treated in Department of Gynecology in the hospital from September 2011 to March 2014 were selected as study objects; then the patients were divided into platinum sensitivity group (62 cases) and platinum resistance group (16 eases) according to the Curative effect of platinum; ELISA was used to detect the levels of serum HE4, annexin A3, mesothelin, and CA125 in 78 EOC patients; immunohistoehemical method was used to detect the expressions of HE4, annexin A3, and mesothelin proteins in EOC tissue; the correlations between HE4,annexin A3, mesothelin and platinum resistance of ovarian cancer were analyzed. Results There were statistically significant differences in the levels Of serum HE4, annexin A3, mesothelin, and CA125 between two groups ( P〈0. 05 ) ; there were statistically significant differences in positive expression rates of serum HE4, annexin A3 and CA125 (P〈0. 05) ; the pairwise comparisons of four kinds of proteins in serum showed positive correlations ( P〈0. 05 ) ; there were statistically significant differences in positive expression rates of HE4, annexin A3, and mesothelin in EOC tissue ( P〈0. 05 ) ; the pairwise comparisons of positive expression rates of HE4, annexin A3, and mesothelin in EOC tissue showed there were statistically significant differences ( P〈0. 05 ) ; HE4, annexin A3, and mesothelin expressions in serum were positively correlated with those in EOC tissue (P〈0. 05) . Conclusion HE4, annexin A3, and mesothelin are involved in occun'ence and development of ovarian cancer, which are closely correlated with platinum resistance ; joint detection of HE4, annexin A3, and mesothclin in serum and EOC tissue can understand the probable mechanism of ovarian cancer platinum resistance, predict
