摘 要:目的:探讨食管癌变过程中细胞周期蛋白B1(CyclinB1)、细胞周期蛋白依赖性激酶1(cyclin-dependent kinases 1,CDK1)的变化及其相关细胞周期调控因子p53、Rb的表达及相互关系.方法:选取35例食管鳞癌组织(同一个病例同时存在鳞癌、上皮内瘤变组织及正常食管黏膜),采用组织芯片及免疫组织化学En Vision二步法,分别观察食管鳞状上皮癌变各阶段Cyclin B1、CDK1、p53和Rb的蛋白表达变化并分析其相互关系.结果:(1)Cyclin B1、CDK1与p53在食管鳞癌组中均呈高表达,且与正常上皮、低级别及高级别上皮内瘤变组间比较均有统计学差异(P<0.05);(2)Rb除鳞癌组与高级别上皮内瘤变组表达率接近外(P>0.05),其他各组间表达率均具有显著统计学差异(P<0.01);(3)Cyclin B1、CDK1、p53及Rb在食管鳞癌组中的表达均呈显著正相关(P<0.01).结论:(1)在食管癌变的过程中,Cyclin B1、CDK1、p53及Rb基因蛋白过表达并协同作用;(2)Rb蛋白的过表达参与了食管上皮内瘤变的演进过程,是食管癌变的早期事件;(3)联合检测Cyclin B1、CDK1、p53和Rb蛋白表达将有助于食管癌的早期诊断及病变发展的评估.AIM:To detect the expression of CyclinBl,cyclin-dependent kinases 1(CDK1),p53,and Rb in esophageal oncogenesis and to analyze their clinical significance.METHODS:Thirty-five cases of esophageal squamous cell carcinoma(each case simultaneously had carcinoma,intraepithelial neoplasia and normal mucosa) were selected and used to prepare tissue chips.The changes of CyclinBl,CDK1,p53 and Rb protein expression in the evolution of esophageal squamous cell carcinoma were observed immunohistochemically and their correlations were analyzed.RESULTS:The positive expression rates of CyclinBl,CDK1 and p53 were significantly higher in esophageal squamous cell carcinoma than in normal esophageal mucosa,low- and high-grade intraepithelial neoplasia(P 〈0.05).The positive expression rate of Rb was similar between esophageal squamous cell carcinoma and high grade intraepithelial neoplasia(P 〉0.05),but differed significantly in other pairwise comparisons(P 〈0.01).CyclinBl,CDK1,p53 and Rb expression had a significantly positive correlation in esophageal squamous cell carcinoma(P〈 0.01).CONCLUSION:In esophageal oncogenesis,CyclinBl,CDK1,p53 and Rb are overexpressed and have synergistic action.As esophageal intraepithelial neoplasia progresses,Rb protein is overexpressed,which is the early event in gastric oncogenesis of esophageal epithelia.The combined detection of CyclinBl,CDK1,p53 and Rb protein expression will contribute to the early diagnosis and evaluation of esophageal carcinoma.
