SDF-1/CXCR4在胃癌不同Borrmann分型中的表达及意义

摘　　要：目的观察不同Borrmann分型胃癌组织中基质细胞因子(SDF-1)及其特异性受体(CXCR4)的表达水平,研究SDF-1/CXCR4生物轴与胃癌不同Borrmann分型的关系及意义。方法回顾性研究173例进展期胃癌标本,按照Borrmann分型分为四组,采用免疫组化分别检测四组胃癌组织中SDF-1及CXCR4的表达水平。结果 (1)SDF-1和CXCR4在胃癌组织中的阳性表达率,明显高于癌旁正常组织的阳性表达率,差异有统计学意义(P<0.05);(2)SDF-1和CXCR4在胃癌组织中的表达水平与肿瘤的病理分级、肿瘤的浸润深度、有无淋巴转移、有无远处转移有密切关系,差异有统计学意义(P<0.05);(3)BorrmannⅣ型、Ⅲ型、Ⅱ型、Ⅰ型中SDF-1及CXCR4的表达水平依次降低,且差异均有统计学意义(P<0.05);(4)SDF-1的表达与CXCR4的表达呈正相关。结论 SDF-1/CXCR4的表达水平与胃癌的Borrmann分型密切相关,在临床中对患者病情评估、预后判断及治疗方面有着广阔的应用前景,有助于判断患者胃癌的恶性程度,为临床胃癌的治疗提供靶向目标。Objective To observe the gastric stromal tissue cytokines （SDF-1） and specific receptor （CXCR4） expression levels in different Borrmann type and research the relationship and significance of SDF-1/CXCR4 biological axis and different Borrmann type of stomach cancer. Methods A retrospective study of 173 cases of advanced gastric cancer specimens was made, according to the Borrmann type were divided into four groups, SDF-1 and CXCR4 expression levels were detected by immunohistochemical in four groups of gastric cancer tissues. Results SDF-1 and CXCR4 expression rate in gastric carcinoma was significantly higher than the adjacent normal tissues, the difference was statistically significant （P〈0.05）. SDF-1 and CXCR4 in gastric cancer expression levels were closely related with tumor histological grade, depth of invasion, lymph node metastasis, distant metastasis, the difference was statistically significant （P〈0.05）. Expression levels of SDF-1 and CXCR4 decreased in turn in Borrmann Ⅳ type, Ⅲ type, Ⅱ type, Ⅰtype, and the differences were statistically significant （P〈0.05）. SDF-1 expression was positively correlated to CXCR4 expression. Conclusion The expression levels of SDF-1/CXCR4 is closely related to Borrmann type of gastric cancer, it has broad application prospects in the clinical assessment of the patient＇s condition, prognosis and treatment, help determine the degree of malignancy in patients with gastric cancer, gastric cancer clinical targeted therapy goals.
