单侧完全性唇腭裂患者上颌前牵引矫治后颌面软组织侧貌变化

摘　　要：目的：探讨生长发育高峰期前的单侧完全性唇腭裂患者矫治后颌面软组织侧貌的变化。方法：选择生长发育高峰期前,凹侧貌单侧完全性唇腭裂患者13例（男6例,女7例）,年龄8.2-10.7岁,经上颌前方牵引矫治1-1.5年,矫治前后拍摄头颅侧位片并测量,采用配对t检验,统计学分析结果。结果：治疗后侧貌明显改变,上颌骨前移,SNA增大（2.22±1.25）°,ANB角增大（4.47±2.15）°,SNB减小-2.25±1.50,差异有显著统计学意义（P〈0.001）,IMPA减小-2.37±4.55,下颌向下向后旋转。MP/SN增大1.69±2.79,Ans-Me增加2.43±1.16mm,差异有统计学意义（P〈0.05）。上颌软组织突度增加（1.65±2.26）°（P〈0.001）,下颌软组织突度减小（2.18±3.18）°（P〈0.005）,上下颌软组织的突度差减小（-3.83±3.71）°,差异有显著统计学意义（P〈0.005）。上下唇突度差减小（-4.21±5.02）mm,差异有统计学意义（P〈0.05）。结论：生长发育高峰期前上颌前牵引矫治凹侧貌单侧完全性唇腭裂患者能促进上颌骨前移,改善上下颌骨关系和颌面软组织侧貌。Objective To investigate the soft tissue profile changes in patients before puberty with complete unilateral cleft lip and palate after maxillary protraction. Methods 13 growing patients with complete unilateral cleft lip and palate were treated with maxillary protraction,lateral cephalometeic films were taken before and after treatment,the changes of soft tissue profile were evaluated by cephalometric analysis and statistics. Results The profile changes after maxillary protraction,maxilla move forward,SNA was increased（2.22± 1.25）°,ANB was Increased（4.47±2.15）°,SNB was decreased - 2.25 ± 1.50（P〈0.001 ）,IMPA was decreased- 2.37 ± 4.55,MP/SN was increased 1.69± 2.79,Ans- Me was increased 2.43 ± 1.16mm（P〈0.05）,S- Ns- Sn was increased（1.65 ± 2.26）° （P〈0.001）,S- Ns- Bs was decreased 2.18 ± 3.18° （P〈0.05）,Sn- Ns- Bs was decreased（- 3.83 ±3.71）° （P〈0.05）.LL- UL was decreased（-4.21 ± 5.02）mm（P〈0.05）. Conclusion After treatment of maxillary protraction,sagittal skeletal intermaxillary relationship improved and soft tissue profile changes singnificantly.
