根管超声荡洗联合氯己叮治疗有瘘型慢性根尖周炎一次性根充的疗效分析

摘　　要：目的:探讨超声荡洗与2%氯已定联合应用有瘘型慢性根尖周炎一次性根充的疗效观察。方法:将186例患者分为两组,实验组用2.5%次氯酸钠超声荡洗与2%氯已定、根充糊剂行窦道搔刮术后一次性根充治疗,对照组则用常规双氧水、生理盐水,根充糊剂行常规根管治疗术多次法进行治疗,1个月、6个月,1年随诊通过临床检查和X线片评估其疗效。结果:实验组1周后牙齿功能恢复正常,2周瘘管消失;6个月后X光片根尖区稀疏区缩小,根尖病变减小,1年后牙齿功能恢复正常,X线片根尖稀疏区缩小,根尖区病变消失,4例失败牙拔除,成功率96.42%。结论:2.5%次氯酸钠根管超声荡洗,2%洗必泰应用有瘘型慢性根尖周炎一次性根管治疗疗效短,效果佳。Objective To observe and investigate the curative effects of the combination of ultrasonic irrigation and 2% Chlorhexidine was used in the root filling process of fistulous chronic periapical periodontitis. Methods 186 patients were divided into two groups.Ultrasonic irrigation by 2.5%Sodium Hypochlorite and 2% Chlorhexidine ＆ AH-Plus were applied after Sinus curettage in the one-time canal treatment for the experimental group.On the other hand, hydrogen peroxide,saline,AH-Plus were used in the conventional pulp canal therapy multiple times on the comparison group.The effects were evaluated by clinical examination and X-ray film after 1 month,6 months and 1 year. Results For the experimental group,one week later,the tooth function returned to normal;2 weeks later,the fistula disappeared;6 months later the sparse area of root tip got narrowed shown in the x-ray film and the periapical lesions decreased.One year later, the tooth function returned to normal,the sparse area of root tip got further narrowed in the x-ray film and the periapical lesions disappeared.4 cases failed and the teeth are pulled out.The overall success rate is 96.42%. Conclusion Ultrasonic irrigation by 2.5% Sodium Hypochlorite,2% chlorhexidine and AH-Plus application in the root filing process of fistulous chronic periapical periodontitis are proved to be effective and fast in the one-time canal treatment.
