胰岛素抵抗与糖尿病性黄斑水肿的相关性研究

摘　　要：目的探讨胰岛素抵抗与糖尿病性黄斑水肿（DME）的关系。方法回顾性分析2013年8月~2015年1月西安市第四医院眼科收治的98例糖尿病性视网膜病变患者。记录患者一般信息、实验室检查及临床情况,检测其胰岛素抵抗情况,运用Logistic回归分析糖尿病性黄斑水肿与胰岛素抵抗指数之间的关系。结果 98例糖尿病性视网膜病变患者中DME患者28例,非DME患者70例。与非DME患者比较,DME患者女性较多见（P=0.045）,糖尿病病程更长（P=0.002）,体重指数较高（P=0.001）,胰岛素抵抗指数水平高（P=0.000）,而高密度脂蛋白胆固醇水平偏低（P=0.007）。Logistic回归分析显示胰岛素抵抗指数与糖尿病性黄斑水肿相关（P=0.000,OR=8.469,95%CI：2.467~29.071）。结论胰岛素抵抗可能与糖尿病性黄斑水肿有关。Objective To assess the association between insulin resistance and diabetic macular edema（DME）. Methods From August 2013 to Janurary 2015, in Department of Ophthalmology, Xi＇an No.4 Hospital, 98 patients with diabetic retinopathy were selected. Routine information, clinical features, and labortary parameters were analysised, and binary logistic regression was used to assess the association between insulin resistance and DME. Results Among 98 patients with diabetic retinopathy, 28 patients with DME, 70 patients without DME. Patients with DME had more female（P = 0.045）, longer duration of diabetes（P = 0.002）, higher body mass index（BMI）（P = 0.001）, higher insulin resistance index（P = 0.000） and lower high density lipoprotein cholesterol（P = 0.007）. Logistic regression model demonstrated that insulin resistance index was associated with DME（P =0.000, OR = 8.469, 95%CI 2.467-29.071）. Conclusion Insulin resistance is associated with DME.
