直肠癌腹腔镜超低位前切除术后的肛肠动力学研究

摘　　要：目的研究直肠癌患者腹腔镜超低位前切除术后的肛肠动力学变化及其与肛门功能的关系。方法选取低位直肠癌患者60例,术后1个月采用肛肠测压仪测定患者肛肠动力学指标;并且评估患者肛门功能;随后,统计分析肛肠动力学指标和肛门功能的相关关系。结果超低位前切除手术后直肠癌患者肛管静息压、直肠最大耐受容量、顺应性以及肛门抑制反射均显著下降(P<0.05),同时肛管直肠静息压(P=0.001、γs=0.493)、直肠最大耐受容量(P=0.008、γs=0.426)、直肠顺应性(P=0.001、γs=0.536)与肛门功能呈显著性正相关;直肠肛门抑制反射和肛管最大收缩压与肛门功能不存在相关性(P>0.01)。结论本研究结果表示直肠癌患者超低位前切除术后肛门功能的变化与肛肠动力学肛管直肠静息压、直肠最大耐受容量、直肠顺应性呈正相关关系,且术后均显著下降。Objective To investigate the relationship between anorectal dynamics and anal function in colorectal cancer patients after the laparoscopic ultra-low anterior resection.Methods 60 patients with low rectal cancer were selected.1 months after the operation,anorectal manometry apparatus was used to determined anal resting pressure,maximal squeeze pressure,maxi-mum tolerable rectal volume,rectal compliance and rectoanal inhibitory reflex;and correlation data and the relationship of the anal function of patients 1 month after operation and anorectal dynamics were analyzed.Results The patients with rectal cancer after the operation of ultra-low anterior resection,anal resting pressure,rectal maximum tolerable volume,rectal compliance and anal in-hibitory reflex decreased significantly （P〈0.05）;Also,anal resting pressure （P=0.001,γs=0.493）,maximum tolerance rec-tal volume （P=0.008,γs=0.426）,rectal compliance （P=0.001,γs =0.536） were significantly positively correlated with anal function;maximal squeeze pressure,rectoanal inhibitory reflex had no no correlation with anal function （ P〉0.01） .Conclusion The change of anal function in patients with rectal cancer after ultra-low anterior resection have positive correlation with anorectal dynamics,especially anal resting pressure,maximum tolerable rectal volume,rectal compliance,all significantly decreased after the operation.
