纳米碳示踪结合多功能保留颈清扫术治疗分化型甲状腺癌临床分析

摘　　要：目的探讨纳米碳淋巴示踪在甲状腺癌中央区清扫中的应用及多功能保留颈淋巴结清扫术中应用的可行性及意义。方法 18例甲状腺癌患者,其中CN0 8例,CN+10例,甲状腺癌患者术中先向癌灶周围注射纳米碳,再行清扫术。CN0 8例行中央区清扫,冰冻提示中央区阳性者加侧后多功能保留颈清扫;CN+10例行中央区加颈部多功能保留淋巴清扫术。术后病理检查黑染组织中有无甲状旁腺及病理淋巴结转移情况。术后感觉测定采用轻触和针刺与健侧比较。结果纳米碳使第Ⅵ区淋巴结黑染,而甲状旁腺不黑染,且清扫黑染组织均未发现甲状旁腺。术后患者颈部、耳廓感觉良好。结论纳米碳淋巴示踪及多功能保留手术疗效确切,在彻底清扫转移灶的同时使甲状旁腺及颈部感觉等多种功能得以保存,但部分患者术后疼痛明显。Objective To explore the feasibility and significance of carbon nanoparticles and multifunctional reservation in cervical lymphadenectomy of thyroid carcinoma.Methods 18 cases of patients with thyroid cancer were retrospectively ana-lyzed.CN ＋10 cases,CN0 8 cases,nano carbon injection were performed before operation.CN0 8 received central area clean-ing,CN ＋received multifunctional retention and neck dissection in the central region.Postoperative pathological examination for parathyroid gland in the black dye and pathologic lymph node metastasis.Postoperative feel determination using touch and acu-puncture were compared with the healthy side.Results Nano carbon makeⅥdistrict lymph node to black dye,and parathyroid not dyed black,And parathyroid gland had not been found in cleaning the black dye,neck and ears feel good after operation.Con-clusion Nano carbon lymphatic tracer and multifunctional reserve operation have significant effect in the treatment of thyroid car-cinoma,the parathyroid gland and neck feel and other functions are protected in the thorough cleaning metastases at the same time,but some patients have significant postoperative pain.
