宫颈上皮内瘤变患者阴道镜多点活检和LEEP术后组织病理学分级结果比较研究

摘　　要：目的比较子宫颈上皮内瘤变(CIN)患者阴道镜多点活检与LEEP术后组织病理学分级结果的差异,并分析其相关原因。方法回顾性分析经LEEP术诊治及阴道镜多点活检110例CINⅠ患者,所有对象术前取宫颈脱落细胞标本进行乳头瘤状病毒(HPV)检查及液基薄层细胞学(TCT)检测,探讨LEEP术后组织病理学分级结果与年龄、分娩方式等一般情况和HPV检测及TCT分类的关联性。结果 110例CIN患者LEEP术后诊断为宫颈黏膜慢性炎(NILM)67例,CINⅠ27例,CINⅡ13例,CINⅢ2例,宫颈早期微浸润癌1例。2LEEP术后组织病理学分级与TCT分类呈正相关(r=0.254,P=0.007)。TCT检查NILM 23例,LEEP术后组织病理学检查为NILM 15例(65.2%);TCT检查为ASCUS 50例,LEEP术后病理检查为NILM 37例(74.0%),CINⅠ10例(20.0%),CINⅡ2例(4.0%),CINⅢ及以上1例(2.0%)。3LEEP术后病理分级与患者年龄、分娩方式及术前HPV感染均无统计学相关性(P>0.05)。结论宫颈脱落细胞标本检查结果,在CIN患者的治疗中可以作为参考指标,具有一定的临床应用价值。Objective To investigate the difference between colposcopy multiple biopsies and histopathological grading results after LEEP conization in cervical intraepithelial neoplasia （ CIN） ,and analyze relevant factors.Methods 110 patients of CINⅠdiagnosed by cervical multipoint biopsy and underwent cervical LEEP conization were retrospectively analyzed.All patients received human papilloma virus（HPV） test and thin prep liquid-based cytology test（TCT） .Results ①Among 110 patients with CINⅠdiagnosed by colposcopy multibiopsy,there were 67 cases of mucosa of chronic inflammation,27 cases of CINⅠ,13 cases of CINⅡ,2 cases of CINⅢ.and 1 case of early micro invasive cervical carcinoma were confirmed by LEEP conization pa-thology.②There was a positive correlation（ r=0.254,P=0.007） ,between LEEP conization pathology and cervical cytology test before LEEP.23 cases diagnosed NILM were affirmed no lesion by conization pathology（65.2%,15/23）.25 cases diagnosed LSIL were affirmed no lesion or low grade lesion（74.0%,37/50）.The statistics showed no significant difference（P〉0.05）in age,number of pregnancy, maternity case, mode of delivery and HPV infection in different groups of pathological results after LEEP.Conclusion Cervical cell specimens test results in the treatment of patients with CIN can be used as a reference index,it has some clinical value.
