摘 要:目的探讨中老年人群高尿酸血症与结直肠腺瘤性息肉(CAP)间的关系。方法调查符合分析条件的3567例45岁及以上的中老年宁波市某石化企业在职和退休员工,根据结肠镜检查结果分为CAP组(486例)和正常对照组(3081例),对入组对象的体重指数、腰围、血压、血脂谱、空腹血糖和尿酸等指标进行分析。分析各组的CAP患病率情况及临床特征,并采用Logistic回归进行CAP患病危险因素分析。结果 CAP检出率为13.6%,其中男性17.0%,女性8.3%,男性检出率明显高于女性(P<0.05);CAP组年龄、腰围、BMI、收缩压、舒张压、低密度脂蛋白胆固醇、空腹血糖、尿酸均高于正常组(P<0.05);CAP组高尿酸血症患病率高于正常对照组,随着尿酸浓度的升高,CAP检出率逐渐升高(P均<0.05);多因素非条件Logistic回归分析,显示性别、年龄、腰围为CAP的独立危险因素,而高尿酸血症与CAP未见明显相关性(P=0.369,OR=1.125,95%CI:0.870,1.453)。结论中老年人群性别、年龄、腰围与CAP关系密切,而高尿酸血症与CAP间未见显著相关性。Objective To explore the relationship between hyperuricemia and colorectal adenomatous polyps( CAP) in middle-aged and elderly population. Methods A total of 3567 employees and retirees aged 45 years old or above who met the analytical conditions in a petrochemical Enterprise of Ningbo city were enrolled and divided into CAP group( n =486) and normal control group( n = 3081). The body mass index( BMI),waist circumference,blood pressure,blood lipid profile,fasting blood-glucose,uric acid were analyzed. CAP prevalence and clinical characteristics of each group were analyzed,and logistic regression was analyzed to predict independent risk factors of CAP. Results The detection rate of CAP was 13. 6%,17. 0% were male and 8. 3% were female( P〈0. 05). The detection rate of male was higher than female significantly. The age,waist circumference,BMI,systolic blood pressure,diastolic blood pressure,LDL cholesterol,fasting glucose and uric acid in CAP group were higher than those in the normal control group,and hyperuricemia prevalence of CAP group was higher than that in the control group( P〈0. 05). The CAP detection rate was gradually increased with the rise of uric acid concentration( P〈0. 05). The multivariate logistic regression analysis showed that sex,age,waist circumference were independent risk factors for adenomatous polyps,and hyperuricemia had no significant correlation with adenomatous polyps( P = 0. 369,OR = 1. 125,95% CI: 0. 870-1. 453). Conclusion The sex,age,waist circumference had relatively close relationship with adenomatous polyps. The detection rate of CAP may not be affected by hyperuricemia in middle-aged and elderly people.
