摘　　要：目的 探讨顺行及逆行两种方式前列腺癌根治术的临床疗效情况.方法 回顾性分析本院2013年2月至2015年2月期间接受开放性前列腺癌根治术的84例患者的临床资料,47例接受耻骨后顺行前列腺癌根治术,37例接受耻骨后逆行前列腺癌根治术,比较分析两组患者的临床差异.结果 两组患者手术顺利,无死亡病例,顺行组术中出血量明显少于逆行组,差异具有统计学意义（P＜0.05）;顺行组切缘阳性例数明显少于逆行组,差异具有统计学意义（P＜0.05）;顺行组长期并发症发生例数明显少于逆行组,差异具有统计学意义（P＜0.05）.结论 耻骨后顺行前列腺癌根治术术中出血少、切缘阳性率低,且长期并发症发生率明显优于逆行术式。Objectives To investigate the differences of clinic efficacy between two kinds of open radical prostatectomy（ORP）.Methods Retrospectively analyze the clinical datas of 84 patients with ORP in our hospital from February 2013 to February 2015.47 cases received ORP and 37 cases received retrograde ORP.The operative outcomes and complications between two groups were recorded.Results There was no peri-operative death and cancer specific death.The estimated blood loss of antegrade group was less than that of retrograde group（P 〈 0.05）;the cutting edge positive rate of antegrade group was less than that of retrograde group （P 〈 0.05）;the complication occurrence of antegrade group was less than that of retrograde group （P 〈 0.05）.Conclusions Antegrade radical prostatectomy provides a low incidence of complication,and is better than retraograde operation with less blood loss and lower cutting edge positive rate.
