盐酸右美托咪定复合芬太尼在微创抽吸术治疗高血压脑出血麻醉中的效果

摘　　要：目的观察盐酸右美托咪定复合芬太尼在微创抽吸术治疗高血压脑出血麻醉中的应用效果。方法将86例拟行微创抽吸术治疗的高血压脑出血患者随机分为2组,对照组(n=43)行丙泊酚复合芬太尼麻醉术,观察组(n=43)行盐酸右美托咪定复合芬太尼麻醉术;比较2组患者不同时间点的血流动力学指标、动脉血氧分压(PaO2)和动脉血二氧化碳分压(PaCO2),同时记录围术期的不良反应。结果观察组的血流动力学稳定性较对照组更佳(P<0.05);观察组T3和T4时刻的PaO2高于对照组(P<0.05),且T3和T4时刻的PaCO2低于对照组(P<0.05);观察组的围术期不良反应发生率低于对照组(P<0.05)。结论盐酸右美托咪定复合芬太尼在微创抽吸术治疗高血压脑出血麻醉中有较好的镇静效果、血流动力学稳定性及较小的呼吸抑制,且具有较高的安全性。Objective To observe the application effect of dexmedetomidine hydrochloride combined with fentanyl to hy- pertensive cerebral hemorrhage anesthesia in minimally invasive aspiration. Methods 86 cases with hypertensive cerebral hem- orrhage treated with minimally invasive aspiration were selected and divided into two groups randomly. The control group（n= 43）was treated with propofol combined with fentanyl anesthesia, and the observation group （n = 43）was treated with dexme- detomidine hydrochloride combined with fentanyl anesthesia. The hemodynamics indicator, PaO2 and PaCO2 of two groups were compared, and the adverse reactions of two groups were recorded. Results The stability of hemodynamics in the observa- tion group was better than that in the control group（P〈0.05）. PaO2 of the observation group at Ta and T4 was higher than that of the control group（P〈0.05）, and PaCO2 of the observation group at Ta and T4 was lower than that of the control group （P〈0.05）. The incidence rate of adverse reactions during perioperative period in the observation group was lower than that in the control group（P〈0.05）. Conclusion Dexmedetomidine hydrochloride combined with fentanyl for hypertensive cerebral hemorrhage anesthesia in minimally invasive aspiration not 0nly has good sedation and good stability of hemodynamics, but also has less respiratory depression and has high safety.
