丁苯酞氯化钠注射液联合双重抗血小板治疗进展性脑梗死的临床疗效观察

摘　　要：目的观察丁苯酞氯化钠注射液联合双重抗血小板治疗进展性脑梗死的临床疗效及不良反应。方法 86例急性进展性脑梗死患者随机分成治疗组和对照组。治疗组给予丁苯酞氯化钠注射液联合阿司匹林、氯吡格雷双重抗血小板治疗;对照组给予双重抗血小板治疗。在治疗前和治疗后第8及第15天行NIHSS评分和Barthel指数评分,观察并监测两组的不良反应。结果与治疗前比较,两组患者治疗后第15天NIHSS评分显著降低,Barthel评分显著升高(P<0.05)。与治疗前比较,治疗后第8天,治疗组NIHSS评分显著下降,Barthel评分显著升高(P<0.05)。治疗组于治疗后第8及第15天比较同期对照组,NIHSS评分显著下降,Barthel评分显著升高(P<0.05)。两组患者均无严重不良反应。结论丁苯酞氯化钠注射液联合阿司匹林、氯吡格雷双重抗血小板治疗进展性脑梗死安全有效。[Objective] To evaluate the clinical beneficial and adverse effects of Butylphthalide and Sodium Chloride Injection combined with dual antiplatelet therapy in treating progressive cerebral infarction. [Methods] A total of 86 patients with acute progressive cerebral infarction were randomly divided into two groups, i.e. treatment group and control group. The treatment group was treated with Butylphthalide and Sodium Chloride Injection combined with Aspirin and Clopidogrel, while the control group was treated by Aspirin and Clopidogrel. The clinical nervous function defect degree and activities of daily living were evaluated by the NIHSS and Barthel index respectively before treatment and on the 8th and 15th d after treatment. The adverse reactions of both groups were recorded and monitored. [Results] The NIHSS score was significantly reduced while the Barthel score was significantly increased in both groups on the 15th d after treatment compared to that before treatment （P 〈 0.05）. On the 8th d after treatment, the NIHSS score was obviously decreased but the Barthel score was greatly improved in the treatment group compared to that before treatment （P〈 0.05）. Compared with the control group, the NIHSS score was significantly decreased and the Barthel score was significantly increased in the treatment group on the 8th and 15th d after treatment （P〈 0.05）. There was no obvious adverse reaction in either group. [Conclusions] Treatment by Butylphthalide and Sodium Chloride Injection combined with Aspirin and Clopidogrel is a safe and effective therapy for progressive cerebral infarction.
