MMP-9、GFAP和FN浓度与脑梗死后出血转化的预测作用

摘　　要：目的 探讨基质金属蛋白酶-9（MMP-9）、胶质纤维酸性蛋白（GFAP）和纤维连接蛋白（FN）浓度对脑梗死后出血转化的预测作用. 方法 随机选择2013年2月至2014年2月74例首发脑梗死患者为观察对象,根据入院1周后脑CT或MRI结果将74例患者分成观察组与对照组,每组37例. 观察组为单纯脑梗死未转化为脑出血,对照组为脑梗死后转化为脑出血. 比较两组MMP-9、GFAP及FN水平. 结果 观察组MMP-9为（ 993.32 ± 234.21）μg/L、GFAP 为（ 2366.35 ±864.47 ） ng/L, FN 浓度为（ 2893.63 ±492.63 ）μg/L;对照组 MMP-9 为（1138.28 ±350.16）μg/L、GFAP为（2996.38 ±951.28）ng/L,FN浓度为（3991.73 ±978.75）μg/L;观察组浓度明显低于对照组,差异有统计学意义（ P〈0.01）. 结论 MMP-9、GFAP及FN水平检测对脑梗死后脑出血有明显的预测作用,在临床治疗过程中可扩大应用.Objective To investigate the predictive effect of concentration of MMP-9, GFAP and FN on hemorrhagic transformation after cerebral infarction .Methods Randomly selected 74 patients with first cerebral infarction from February 2013 to February 2014 as observation objects , and divided them into observation group and control group , with 37 cases in each group , according to the CT or MRI results .The observation group was not transformed into pure cerebral infarction cere-bral hemorrhage , and the control group was transformed into cerebral hemorrhage after cerebral infarction .The matrix metallo-proteinase -9（MMP-9）, glial fibrillary acidic protein（GFAP） and fibronectin（FN） levels were compared.Results In obser-vation group, MMP-9 was （993.32 ±234.21）μg/L, GFAP （2366.35 ±864.47） ng/L, the concentration of FN （2893.63 ± 492.63）g/L;MMP-9 of the control group was （1138.28 ±350.16）μg/L, GFAP （2996.38 ±951.28） ng/L, the concen-tration of FN （3991.73 ±978.75）g/L;the indicators above were all significantly lower in observation group than those in control group（P〈0.01）.Conclusions The detection of MMP-9, GFAP and FN levels has significant forecast effect on cerebral infarction cerebral hemorrhage and can expand the application in the course of clinical treatment .
