摘 要:目的:采用体外和临床实验评价粘结剂不同涂布方式对树脂充填物修复效果的影响。方法:将90个离体前磨牙,按照粘结剂Clearfil S Bond不同的涂布方式分为3组(n=30)。A组粘结剂涂布1次,静置15 s,轻吹5 s,固化;B组粘结剂涂布1次后,再次蘸粘结剂连续涂布第2次,静置15 s,轻吹5 s,固化;C组粘结剂涂布1次,静置15 s,涂布第2次,静置15 s,轻吹5 s,固化;均用Z350树脂充填修复,制备粘结树脂微拉伸试件,进行粘结强度测试。临床研究中将150个前磨牙楔形缺损患者按以上方法分为3组(n=50),观察评估不同涂布方式的粘结效果。结果:A、B、C组的微拉伸强度(MPa)分别为10.2±2.1、32.1±3.8和23.3±6.7(B组与A、C组相比P<0.05)。临床观察A、B、C组修复体脱落率(%)分别为6、2、和4(B组与A、C组相比P<0.05);修复体边缘不密合率(%)分别为10、2、和8(B组与A、C组相比P<0.05);修复体变色均为2%;继发龋均为4%。结论:粘结剂Clearfil S Bond连续涂布2次固化1次的涂布方式粘结效果好,可在临床推广。AIM: To study the effect of different coating methods on the restoration effect of composite resin filling material. METHODS: 90 extracted human premolars were prepared into the samples for micro- tension test and divided into 3 groups( n = 30). Clearfil S Bond was spread onece in group A,successively twice in group B,twice with 15 s standing interval in group C,kept standing for 15 s and tootling for 5 s,then Resin Z350( 3M) was adhered to the samples. The bonding strength of the composite resin to the tooth samples was tested using a micro tensile test machine. The clinical observation was performed to evaluate the bonding effect of the composite resin to the tooth in the restoration of the wedge shaped defect in 150 cases with 50 in each group( A,B and C) using above-mentioned methods respectively. The patients were followed up for 1 year. RESULTS: The microtensile strength( MPa) of group A,B and C was 10. 2 ± 2. 1,32. 1 ± 3. 8 and 23. 3 ± 6. 7 respectively( B vs A or C,P〈0. 05). Clinical observation showed that the prosthesis loss( %) was 6,2 and 4 in group A,B and C respectively( B vs A or C,P〈0. 05); unadaptation( %)10,2 and 8 in group A,B and C respectively( B vs A or C,P〈0. 05); discoloration( %) was 2 and secondary caries( %) was 4 in all groups. CONCLUSION: The method of double coating of the adhesive followed by single light curing can be widely applied for the bonding of composite resin Z350 to teeth by adhesive Clearfil S Bond.
