下颌骨升支“箱盖式”入路在咽旁间隙肿瘤手术中的应用

摘　　要：目的观察下颌骨升支"箱盖式"入路在咽旁间隙肿瘤手术中的应用效果。方法 55例咽旁间隙肿瘤患者分为观察组28例与对照组27例,观察组选择下颌骨升支"箱盖式"入路,对照组采取颈侧入路,随访2年,比较两组手术疗效与并发症发生率。结果两组恶性肿瘤患者1年及2年生存率比较,差异均无统计学意义;观察组良性肿瘤患者复发率为3.6%,低于对照组(22.2%),差异具有统计学意义(P<0.05)。观察组并发症发生率为10.7%(3/28),低于对照组的33.3%(10/27),两组比较,差异有统计学意义(P<0.05)。结论下颌骨升支"箱盖式"入路在咽旁间隙肿瘤中的应用可更充分暴露术野,利于肿瘤切除,复发率低,损伤小、术后并发症少,值得推广应用。Objective To observe the effect of the box-lid type approach of mandibular rumas in the operation for parapharyngeal space tumors. Methods 55 cases of patients with pharyngeal space tumors were divided into observation group （28 cases） and control group （27 cases）. The observation group was treated with the box-lid type approach of mandibular rumas while the control group was treated with the ateral approach to cervical spine. The two groups were followed up for 2 years. The curative effect and the incidence rate of complications were compared between the two groups. Results There were no significant differences in 1 and 2-year survival rates between the two groups of patients with malignant tumors; The recurrence rate of the benign patients in the observation group （ 3. 6% ） was lower than that of the control group （ 22. 2% ） （P 〈 0.05 ）. The incidence rate of complications in the observation group （33.3%） （3/28） was lower than that of the control group 33.3% （10/27） （P 〈 0.05 ）. Conclusion The box-lid type approach of mandibular rumas during the operations for parapharyngeal space tumors can fully exposed the surgery field. It is conductive to tumor resection, with low recurrence rate, little injury and few postoperative complications. It is worth promotion.
