eIF4A在卵巢上皮性癌中表达及其与预后的关系

摘　　要：目的:检测真核细胞起始因子4A(e IF4A)在不同卵巢组织中的表达,探讨其临床意义及其与预后的关系。方法:利用荧光实时定量PCR及Western blot法检测42例卵巢上皮性癌(EOC)组织、15例卵巢交界性上皮性肿瘤组织、15例卵巢良性上皮性肿瘤组织和20例正常卵巢组织中e IF4A mRNA及蛋白的表达情况,同时采用免疫组化SP法检测123例EOC石蜡切片中e IF4A蛋白表达情况,并评估影响患者总生存率的高危因素。结果:EOC中e IF4AmRNA及蛋白表达显著高于交界性、良性及正常卵巢上皮组织(P<0.05);交界性上皮肿瘤组织明显高于良性及正常卵巢上皮组织(均P<0.05);卵巢良性上皮性组织略高于正常卵巢组织,但差异无统计学意义(P>0.05)。EOC中e IF4A表达与FIGO手术病理分期、组织学分级、淋巴结转移及腹水情况有关(P均<0.05),而与年龄及组织学类型无关(P>0.05)。Kaplan-Meier生存曲线显示,e IF4A蛋白高表达者的总生存率明显低于低表达者(Log-Rank检验,P<0.05)。COX比例风险模型分析发现,临床病理分期、组织学分级及术后有无残余瘤灶是EOC患者的独立预后因素(P<0.05)。结论:e IF4A在EOC的发生进展中可能起重要作用,有可能成为判断预后的指标之一。Objective： To detect the expression of e IF4 A in different ovarian epithecial neoplasisa tissue and its relationship between cliniclpathological characteristics and prognosis.Method： The expression of e IF4 A was detected by quantitative real-time PCR and Western blot analysis in epithelial ovarian carcinoma tissue（ EOC,n = 42）,borderline epithelial neoplasia tissue（ n = 15）,benign epithelial neoplasia tissue（ n = 15） and normal ovarian tissue（ n = 20） respectively. The expression of e IF4 A protein in EOC paraffin-embedded tissue（ n = 123） was examined by immunohistochemistry. Result： The expression of e IF4 A mRNA and protein was significantly increased in EOC than those in the patients with borderline epithelial neoplasia tissue,benign epithelial neoplasia tissue and normal ovarian tissue（ P 0. 05）. It was significantly higher in borderline epithelial neoplasia tissue than that in benign epithelial neoplasia tissue and normal ovarian tissue（ P 0. 05）. It had no significant statistical difference between benign epithelial neoplasia tissue and normal ovarian tissue（ P 0. 05）. In EOC,the expression of e IF4 A was correlated with the FIGO stage（ P 0. 05）,histological grade（ P 0. 05）,lymphatic metastasis（ P 0. 05） and ascites（ P 0. 05）. However,it had no relationship with histological type（ P 0. 05） and age（ P 0. 05）. Kaplan-Meier survival curves indicated that patients with high e IF4 A expression had lower overall survival rates（ OS） compared to those with low e IF4 A expression（ Log-Rank test,P 0. 05）. Multivariates COX preportional hazards model demonstrated that clinical stage,histological grade and postoperative residual lesions were the independent prognostic factores for EOC（ P 0. 05）. Conclusion： The e IF4 A plays a certain role in the oncogenesis and malignant progression of EOC. The EOC patents with high e IF4 A expression might have poor prognosis.
