高压氧辅助治疗高原急性缺血性脑卒中的效果

摘　　要：目的 探讨高压氧辅助治疗高原急性缺血性脑卒中的效果.方法 选择高原急性缺血性脑卒中90例,年龄40~70岁.采用随机数字表法,将患者随机分为2组（n=45）：对照组（C组）,高压氧治疗组（H组）.2组患者均给予保持呼吸道通畅、吸氧、营养支持等常规治疗,在此基础上,C组：阿司匹林肠溶片、阿托伐他汀钙经口服或胃管注入,单唾液酸四已糖神经节苷脂钠经静脉滴注,每日1次;H组：在对照组治疗基础上,采用高压氧治疗,每日1次,10次为1个疗程,连续治疗2个疗程,共20 d.20 d后评定2组患者神经功能改善情况和治疗效果.结果 与治疗前比较,治疗后20 d 2组NIHSS评分均降低;与C组比较,H组NIHSS评分降低（P〈0.05）.H组总有效率为93.3%,C组为82.2%;与C组比较,H组总有效率升高（P〈0.05）.结论高压氧治疗可有效改善高原急性缺血性脑卒中患者的神经功能缺损程度,辅助联合药物治疗,疗效更佳.Objective To investigate the effects of hyperbaric oxygen therapy in acute cerebral ischcmic stroke at high altitude localities. Methods Ninety patients with acute cerebral ischemic stroke, aged 40-70 yr, were randomly divided into two groups （n=45 each） ： control group （group C） and hyperbaric oxygen therapy group （group H） . Two groups of patients were treated with maintaining airway patency, oxygen and nutrition support. Based on these, the patients in group C were treated with aspirin enteric-coated tablets, atorvastatin calcium tablets and monosialotetrahexosylganglioside sodium injection, while the patients in group H were treated with hyperbaric oxygen therapy on the basis of group C. Once-daily therapy and 10 times as one course of treatment. The neurologic impairment score and the therapeutic efficacy were compared between the two groups afler two courses of treatment. Results After the treatment of twenty days, the neurologic impairment scores of the two groups were significantly lower than before. Compared with group C, the neurologic impairment score in group H was significantly lower （P 〈 0.05） . The total effective rates in groups H and C were 93.3% and 82.2% , respectively. The total effective rate in group H was higher than that in group C （P 〈 0.05） Conclusion Hyperbaric oxygen therapy combined with medical treatment can obviously improve neural function of patients with acute cerebral ischemic stroke and increase the total effective rate.
