胸腔镜下儿童纵隔肿瘤切除术九例分析

摘　　要：目的：总结胸腔镜手术切除儿童纵隔肿瘤的技巧。方法对9例纵隔肿瘤患儿采用胸腔镜下肿瘤切除治疗。结果全部患儿均顺利在腔镜下完成手术，手术时间60~200分钟，平均（133.3±58.3）分钟。术中出血量﹤10 ml。无因为出血以及创伤而中转开胸患儿。无围手术期死亡患儿。术后无因神经损伤导致隔神经麻痹以及 Horner 综合征等，无气管以及食管瘘的发生。随访未发现肿瘤复发。结论如选择合理的手术入路，胸腔镜切除儿童纵隔肿瘤手术安全可行。Objective To evaluate the techniques of thoracoscopic resection for mediastinal mass. Methods The clinical data of 9 cases of thoracoscopic surgery for mediastinal mass were reviewed retrospectively. Results All mediastinal masses were completely excised. The operating duration ranged from 60 to 200 min,with an average of（133. 3 ± 58. 3）min. Intraoperative bleeding was less than 10 ml. No case was converted to open surgery due to bleeding or trauma. No patient died during the perioperative period. Major complications such as phrenic nerve paralysis,Hornerˊs syndrome,tracheal fistula,or esopha-geal fistula were not observed. No tumor recurrence occurred during the follow-up period. Conclusion With selected approaches and techniques,thoracoscopic resection for pediatric mediastinal mass can be safely performed.
