83例颈动脉内膜剥脱术的手术配合

摘　　要：目的总结2009年4月-2013年12月83例颈动脉内膜剥脱术的手术配合，有针对性地预防和控制术中并发症，保证患者顺利完成手术，改善患者生活质量。方法进行统计的83例手术患者男性65例，女性18例，年龄40～78岁，平均（59～7．6）岁，其中狭窄程度均在75％以上，既往有高血压83例，冠心病26例，糖尿病34例。83例均在全麻下行颈内动脉内膜剥脱术，1例在荧光造影下找到斑块。结果术后80例患者的临床症状完全消失，2例明显好转，1例突发脑出血死亡。术后随访12～18个月，均未发现再狭窄，患者症状明显改善，肌力恢复，TIA消失，70例能正常工作和生活，6例偶有头晕，6例主诉术后颈部活动受限。结论手术配合要求器械护士要严格执行无菌操作，熟悉解剖位置，了解手术步骤，合理安排器械布局，及时准确传递器械，缩短手术时间，减少并发症的发生。巡回护士密切关注患者生命体征，积极主动做好手术配合，是整个手术取得成功的重要保证。Objective To summarize the surgical cooperation of 83 cases of carotid artery intima stripping in April 2009-2013 in December, and to prevent and control complications, ensure the patient＇s quality of life. Methods 83 cases of 65 patients, 18 women, 40 patients, aged 78 to years, average （59＋7.6） years old, and the degree of stenosis was more than 75%, 83 cases of hypertension, 26 cases of coronary heart disease, 34 cases of diabetes mellitus. 83 cases were all under general anesthesia, and I cases of carotid artery intima were removed, and the plaques were found in the fluorescence imaging. Results The clinical symptoms disappeared completely in 80 patients, 2 cases were improved, 1 cases died of cerebral hemorrhage. Postoperative patients were followed up for 12 to 18 months, were not found restenosis, the symptoms were improved obviously, muscle recovery, the disappearance of TIA, 70 cases can normal work and life, 6 cases had occasional dizziness, 6 cases complained of postoperative neck movement is limited. Conclusions Surgical operation with the requirements of equipment nurses to strictly implement aseptic operation, familiar with the anatomical position, understand the operation steps, reasonable arrangements for the layout, timely and accurate delivery of equipment, shorten the operation time, reduce the occurrence of complications. The circuit nurses pay close attention to the vital signs of the patients, and actively cooperate with them, and are the important guarantee for the success of the operation.
