细胞角蛋白19 mRNA和肺特异性X蛋白mRNA在非小细胞肺癌患者外周血微转移检测中的意义

摘　　要：目的探讨细胞角蛋白19 mRNA（CK19 mRNA）和肺特异性X蛋白mRNA（LUNX mRNA）在非小细胞肺癌（NSCLC）患者外周血微转移检测中的临床意义。方法采用逆转录PCR反应检测64例NSCLC患者、20例肺良性疾病患者及30例健康者外周血中CK19 mRNA和LUNX mRNA的表达。结果 CK19 mRNA、LUNX mRNA在NSCLC组表达阳性率分别为48.4%和56.3%,肺良性病变组为5.0%和0.0%,健康对照组均未见表达。NSCLC患者CK19 mRNA和LUNX mRNA表达在3期和4期外周血微转移中的阳性率显著高于1期~2期,差异有统计学意义（P〈0.05）。2个指标联合检测的敏感度、特异度、阳性预测值、阴性预测值分别为60.9%、100.0%、100.0%、66.7%。结论 CK19 mRNA和LUNX mRNA可作为NSCLC微转移较为敏感和特异的指标,联合检测可提高检出率。Objective To explore the clinical significance of detecting cytokeratin 19（ CK19） mRNA and lung specificity protein X（ LUNX） mRNA in peripheral blood micrometastasis of patients with non-small cell lung cancer（ NSCLC）. Methods The peripheral blood of 64 cases of NSCLC,20 cases of lung benign disease and 30 cases of healthy controls were detected for CK19 mRNA and LUNX mRNA by reverse transcription polymerase chain teaction（ RT-PCR）. Results The positive rates of CK19 mRNA and LUNX mRNA expression was respectively 48. 4% and 56. 3% in NSCLC group,that was 5. 0% and 0. 0% in lung benign disease group,and that was not detected in healthy controls. It was significantly higher in patients peripheral blood micrometastasis with 3 stage and 4 stage than that in 1 stage and 2 stage of patients with NSCLC,and the difference was statistically significant（ P〈0. 05）. The sensitivity,specification,positive predictive value and negative predictive value were respectively 60. 9%,100. 0%,100. 0% and 66. 7% within the 2 indicators joint detection. Conclusion CK19 mRNA and LUNX mRNA can be a sensitive and specific marker for micrometastasis in NSCLC,so joint detection can improve the detection rate.
